Thank you for your letter of July 24, 1986, in response to our letter and the attached Notice of Violation dated June 24, 1986. As a result of our review, we find that additional information, as discussed with your Messrs. G. E. Wuller and Bob Lailhauque (during a telephone call on August 13, 1986), is needed. Specifically, the following information is needed:

Violation 8601-01 What percent of the Waterford 3 Steam Electric Station (W35ES) active badged employees having unescorted access to the protected area also have unescorted access to the vital areas of the switch gear room A, A/B, and B?

What are the good cause criteria your staff uses for granting unescorted access to vital areas for general familiarization and non-work.related activities?

How much time do you allow for general f amiliarization with your vital area ,

access authorization; i.e., how many days per year do you provide unescorted vital area access for non-work related activities or general familiarization?

Violation 8601-02 Is a security officer posted at a door / portal prior to a normally locked and alarmed door / portal being open when it is known by the maintenance person or

,

craftsman that it will be opened for a period of time which will be longer j than that time required for normal access?

Is a normally locked and alarmed door / portal returned to a locked and alarmed mode after the craftsman or maintenance person executes normal entry, or is

the door / portal left only with an alarm but no lock, or is the door locked with the alarm accessed?

Violation 8601-03 What are the six point criteria established in Nuclear Operations Executive

What was the interval of time in hours or days that door 266 was unlocked?

Was stealth used to open door 266 or was an authorized key used to open door 266?

If an authorized key was used, was the key in the possession of the authorized key custodian?

Who was the last person to exit door 266? What was the elapsed time between this exit and the alarm received?

i Who or what caused the alarm on door 266?

Are the balanced magnetic switches so insensitive that a false presumption of a door being locked can be made because there is no alarm?

Do security officers responding to an alarm and unlocked door routinely accept statements from personnel outside the alarmed area that were unaware of the cause for an alarm?

Are unexplained alarms with unlocked doors routinely and arbitrarily documented

as an unknown cause?

Was there an investigation initiated after it was discovered that the security officer failed to document the alarm properly?

Do you routinely check for the root cause of doors / portals found unlocked?

Violation 8601-07 Was there a review made of all vital area barriers to assure that no other vital barriers had openings as the specific one mentioned in this violation?

- If so, what were the results of such a review? Confirm that all other inner compartment vital area barriers met or exceeded 10 CFR 73.2(f)(2) criteria and

did not have openings exceeding the 96 square inch criteria for such openings?

- vital areas to assure that no other vital area locking device could so easily be defeated?

If so, confirm that the locking devices meet or exceed the requirements for sufficient strength and manipulation resistance as defined in 10 CFR 73.2(m)

and assure the high assurance objective of 10 CFR 73.55(a).

Please provide the supplemental information within 30 days of the date of this letter.
