Sheet 1 of 1 LP&L SUPPLEMENTAL RESPONSE TO VIOLATION NO. 8605-01 NRC Request for Additional Information In response to the LP&L response to Violation No. 8605-01, the NRC requested that additional information be provided, "Specifically, you should address corrective actions associated with performance of the work without an adequate RWP and test procedure as required by the NOV."

LP&L Supplemental Response (1) Corrective Action That Will Be Taken Regarding the lack of an adequate RWP, LP&L will issue two letters.

The first letter will be issued to all plant personnel and they will be directed to ensure that they keep Operations personnel cognizant of work in progress and that Health Physics personnel are made aware of changes to the scope of work covered under a specific Radiation Work Permit particularly if such change increases the potential for changing the radiological conditions in the work area.

The second letter will be issued to appropriate Health Physics personnel to remind them of the importance of ensuring that they are knowledgeable of the total scope of a job to be performed inside the RCA and that they evaluate potential adverse consequences of such work particularly with regard to the need for additional radiological monitoring of the work area.

A test procedure for Station Modification SM-818 has been develcped which includes checks for leakage and crossed lines as well as provides for an integrated functional test of the system. This test procedure is expected to be reviewed and approved by PORC before August 22, 1986.

(2) Date When Full Compliance Will Be Achieved The two letters will be issued by September 15, 1986 and this will achieve full compliance.
