Gentlemen:
This refers to the inspection conducted by Messrs. W. F. Smith and S. D. Butler
-
of this office during the period September 5 through October 1, 1990, of
activities authorized by NRC Operating License NPF-38 for Waterford Steam
Electr .'c Station, Unit 3, and to the discussion of our findings with
Mr. J. R. McGaha and other members of your staf f at the conclusion of the
inspection.
Areas examined during the inspection included onsite followup of events,
monthly maintenance observation, monthly surveillance observation, operational
safety verification, followup of previously identified items, licensee event
report followup, and engineered safety feature system walkdown.
Within these
areas, the inspection consisted of selective examination of procedures and
'
representative records, interviews with personnel, and observations by the
inspectors.
The inspection findings are documented in the enclosed inspection
report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements.
Consequently, you are required to respond to
these violations, in writing, in accordance with the provisions of
Section 2.201 of the NRC's " Rules of Practice," Part 2. Title 10, Code of
'
Federal Regulations.- Your response should be based on the specifics contained
in the Notice of Violation enclosed with this letter.
A weakness in the area of postmaintenance testing was identified while the
inspector was observing maintenance on an atmocpheric steam dump valve.
A
surveillance test was specified that was not suitable for the existing plant
conditions (i.e. , a cold shutdown test was specified Phile the plant was at
full power).
Even_though the shift supervisor caught the error before
authorizing the work to start, the resultant delay unnecessarily prolonged the
-safety equipment outage.- Problems continue to be identified in the area of
postmaintenance and postmodification testing.
A Notice of Violation was issued
on August 1, 1990 (NRC Inspection Report 50-382/90-15), citing similar
circumstances for a postmodification test.
Since your corrective actions have
not been fully implemented, an additional Notice of Violation is not being
issued.
Based on discussions with the General Manager, Nuclear Operations, we
understand that the corrective actions for the previous violation will include

report.
We have also examined actions you have taken with regard to previously
identified inspection findings. The status of these items is identified in
paragraph 7 of the enclosed report.
The response directed by this letter and the accompanying Notice is not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, PL 96-511.
