Dear Mr. Johnson:

This letter is written in reponse to your letter of October 22, 1986, which transmitted Inspection Report STN 50-482/86-17. As requested, the violation (482/8617-01) identified in the Inspection Report is being addressed in four parts.

(a) The reason for the violation if admitted; (b) The corrective steps which have been taken and the results achieved; (c) Corrective steps which will be taken to avoid further violations; and (d) The date when full compliance will be achieved.

. Violation (482/8617-01): Failure To Perform Activities In Accordance With Specified Procedure Finding: Technical Specification (TS) 6.8.1 requires that, " Written procedures shall be established, implemented, and maintained covering . . . a. The applicable procedures recommended in Appendix A of Regulatory Guide (RG) 1.33, Revision 2, February 1978."

Appendix A of RG 1.33, Revision 2, Februrary 1978, states that "The following . . . safety-related activities. . . should be covered by written procedures . . . 1.d. Procedure adherence . . . .

,   ADM 01-001,   Revision 12, April 1, 1986, " Introduction To Wolf Creek Generating Procedures," Section 4.2.4, states that,    "Those participating in any activity shall be made aware of,    and use, proper and current instructions,   procedures, drawings, and engineering'

requirements for

performing the activity as applicable." ,

ADM 02-300, Revision 7, May 20, 1986, " Surveillance Testing," Section 5. 4,

Step 5.4.1 states, "Each test performer is responsible for the. effective

.

conduct of each assigned surveillance activity . . . ." These { responsibilities include: " Verifying prior to its use that a procedure is the current revision."

> Contrary to the above, on July 18, 1986, the NRC inspector observed during ' records review that during the performance of STS CH-008A, " Emergency Diesel Fuel Storage Tank," on June 1, 1986, that the data sheet for STS CH-008B,

  "Dnergency Diesel Fuel Storage Tank," Revision 3 was used instead of the appropriate Revision 4, which had been issued on May 14, 1986. The records review indicated that the test performer failed to use the correct procedure

' and revision, even though he had signed in Section 1 of the cover sheet that he had used the correct procedure and revision. Further, the poet- - 4 performance review cycle failed to discover this error.

Reason For Violation If Admitted: Failure to verify use of the correct procedure and revision by the chemistry i technician who signed the verification portion in Section 1 of the Surveillance Test Routing Sheet was due to unfamiliarity with the ' requirement to verify the current revision with the control room and in - , attention to detail in not realizing he was using the data sheet CH-008B-instead of CH-008A which are identical in format. The technician , incorrectly performed the verification using the procedure obtained from the chemistry group file cabinet.

Corrective Steps Which Have Been Taken and Results Achieved: l The technician has been counseled by his supervision regarding the use of ' the correct procedure and proper method of verification for current ' revision. The chemistry group file cabinet has been purged of all uncontrolled data sheets to prevent use of an out of date revision.

Additionally, an interoffice correspondence (KWOLKW 86-037, dated 9/25/86) was sent to members of the chemistry staff from the Radiochemist Chemistry Supervisor regarding the use of improper and out of date procedures and forms.

' Since the occurrence of this event, no further incidents of this nature have 1 been caused by the chemistry staff.
