Subj: Response to Inspection Report STN 50-482/86-16

Dear Mr. Johnson:

This letter is written in reponse to your letter of July 25, 1986, which transmitted Inspection Report STN 50-482/86-16. As requested, the violation (482/8616-01) identified in the Inspection Report is being addressed in four parts.

(a) The reason for the violation if admitted; (b) The corrective steps which have been taken and the results achieved; (c) Corrective steps which will be taken to avoid further violations; and (d) The date when full compliance will be achieved.

Violation (482/8616-01): Failure To Adequately Isolate Safety-Related Equipment Prior To Maintenance Pinding:

Technical Specification (TS) 6.8.1 requires that, 'Mritten procedures uhall '

be established, implemented, and maintained covering . . . a. The applicable procedures recommended in Appendir A of Regulatory Guide (RG) f 1 33, Revision 2, February 1978."

Section 9.a of RG 1 33, Revision 2, February 1978 states that, . . .

maintenance that can affect the perfomance of safety-related equipment should be properly proplanned and performed in accordance with written procedures, dooumonted instructions, or drawbgs appropriate to the circumstances.

Contrary to the above, on June 19, 1986, maintenance activities being performed on ulve EN HV-15 in accordance with Work Request (WR) 07073-85 caused the unanticipated actuation of some engineered safety features components due to the failure to adequately isolate (i.e., preplan.the maintenance - motor de-energized) the valve motor prior to performing maintenance on it.

Reason For Violation If Admitted:

'

This Engineered Safety Features equipment actuation was initiated by the inadvertent grounding of two rotor contacts while preventative maintenance was being performed on the isolation valve between the containment spray additive tank outlet and the "A" train of containment spray. Although the valve had been electrically isolated, the feedback signal to the Solid State Protection System actuated two slave relays when the rotor contacts were inadvertently grounded. Isolating the rotor contacts in this particular valve is not practical without creating additional potential for inadvertent Engineered Safety Peatures equipment actuation due to system design. The cause of this event was a cognitive personnel error by non-licensed utility personnel, who inadvertently grounded the rotor contacts.

Corrective Steps Which Have Been Taken and Results Achieved:

A training session has been held for electrical craftsmen to discuss this

event and to identify the proper actions to be taken when working on this type of valve. A review was performed to identify other valves which have the potential for causing similar ESP actuations and twelve (12) such valves were identified.

Corrective Steps Which Will Be Taken To Avoid Further Violations:

Procedure MPE E00P-02 is being revised to alert personnel to the extreme oaution necessary when working on this valve and twelve other valves which have been identified as having the potential for causing similar ESP actuations.

The Date When Pull Compliance Will Be Achieved:

Full compliance will be achieved by September 30, 1986.
