Violation 482/9005 02 concerned an inoperable turbine driven auxiliary feed
!
pump which resulted from an improperly set speed controller.
Although the
corrective actions.taken to avoid further violations addressed this specific example, the corrective action did not appear to address the potential for other safety-related equipment to be similarly effected.
Responses i
During a review of the ESF status panel, no other instances were identified I
.
where the mispositioning of ESF equipment controllers would not be l
annunciated on the ESF status panel.
Annunciation on the ESF status panel of equipment controller mispositioning clearly identifies to the operators the status of the equipment, allowing appropriate actione to be determined
!
i and taken.
J Even though mispositioning of the speed controller (FC HIK 313A) for the TDAFWP governor control valve is not directly annunciated on the ESF status

l panel, there is an alarm window on main control board 128-E,
'TD AFP SPD l
HILO'.. which becomes active when the TDAFWP mechanical trip and throttle
'
valve, FC HV-312 is opened. This window will alert the operator if the pump
.
is started and the speed setting is not correct.
The alarm response
,
procedure for this window. ALR 00 128E.
'TD AFP SPD HILO' requires that the
/
operator immediately ensure that the TDAFWP governor control valve speed controller (FC HIK-313A) is at its normal output setting.
Although this alarm function cannot prevent mispositioning of the speed controller it.does l'
provide the ability to promptly return the speed controller to the J
i.
appropriate setting if the,TDAFWP is started.

l In addition, a change has been made to ADM 02 005,
' Reactor Operators
'
l Qualifications and Responsibilities' clearly stating the need for the i
Reactor Operator to be cognizant of plant conditions.
This change was made to address an NRC concern regarding the Reactor Operator's awareness of L
plant conditions.
