This letter refers to the inspection conducted by Mr. M. E. Skow and

Ms. L. L. Gundrum of this office during the period- August 1 to September 14,
i
1990. This inspection pertained to activities authorized by NRC Operating.

License No. NPF-42 for the Wolf Creek Generating Station, and our findings were

discussed with Mr. F. T. Rhodes and other members of your staff at the
conclusion of the inspection,
i
Areas examined during the inspection included plant status, operational safety
verification, monthly surveillance observation, and monthly maintenance
observation. Within the,e ;.reas, the inspection consisted of selective
examination of procedu . and. representative records, interviews with
personnel, and observations by the inspectors. The inspection findings are
documented in the enclosed inspection report.
'
During this inspection, it was found that certain of your activities were in
violation of NRC requirements.
The violations represent an increasing
regulatory concern for the control of work activities and the corrective; action-
process at the Wolf Creek Generating Station. A test of the main generator
.
voltage regulator was conducted without an approved procedure and without
'
having been reviewed for the potential effect on safety related equipment.
The
i
test nearly resulted in the loss of power to the two safety-related busses with-
!
the reactor at 50 percent power and one emergency diesel generator out of-
service. The overall significance of the induced transient was apparently not
recognized by the operators and not reported as required by your corrective
action program. The failure to perform the required safety . review of the test
was later recognized by a quality assurance engineer. However, the.overall
safety significance of this event was again not recognized by your staff, nor
was an encompassing review performed to identify all the corrective' actions
-that should have been taken.
During a management meeting on September 11',
-
1990, your staff did not demonstrate a-clear understanding of-the event. _A
subsequent teleconference was required to clarify the information'provided.

Two other events involving inadequate work control were identified by your
staff. The first event involved scaffolding which had been erected around the

auxiliary feedwater (AFW) system.
The scaffolding was found to inhibit the
movement of an AFW suction line check valve.
The investigation and corrective
actions as documented in the initial licensee event report did not indicate
that a thorough evaluation of the event had been performed.
The second event
involved the unauthorized bypassing of an ef fluent radiation monitor. A small
amount of contaminated liquid was pumped out of a turbine building sump and
released without having been monitored.
The examples contained in tiie Notice of Violation were based on the staff's
evaluation of the main generator voltage regulator test. The examples were
considered for applicable enforcement discretion.
However, the failure of
programmatic controls to prevent the above events, and then to evaluate and
implement prompt correct action, indicates that cited violations are
appropriate.
Consequently, you are required to respond to these violations, in
writing, in accordance with the provisions of rection 2.201 of the NRC's " Rules
of Practice," Part 2, Title 10, Code of Federal Regulations.
Your response
should be based on the specifics contained in the Notice of Violation enclosed
with this letter.
In addition, your response should include your overall
actions to correct the apparent continuing work control problems and an
assessment of the effectiveness of your corrective action program.
The response directed by this letter and the accompanying Notice is not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, PL 96-511.
