Subject: Diagnostic Team Inspection - Combined Inspection Report Nos.

50-277/86-12 and 50-278/86-13

During the period from June 18, 1986, to July 3, 1986, ten members of a Region I

team conducted a special diagnostic safety inspection of operational and other

activities at the Peach Bottom facility. The overall objective of the inspection

was to obtain a more complete understanding of the underlying reasons for the

poor performance described in the June 1986 SALP report, and to ascertain

whether they could have an adverse impact on the safety of plant operations.

The results of this inspection are documented in the enclosed inspection report.

In this, as in every inspection conducted by Region I, we assessed whether the

facility was being operated safely. During the inspection, we found no evi-

dence that the plant was being operated unsafely. Plant personnel and first

line supervisors were found to be both knowledgeable and conscious of nuclear

safety. Notwithstanding this conclusion, we believe that substantial improve-

ments in performance are necessary at your facility before Region I inspection

activities can be reduced. This confirms the conclusions reached in the June

1986 SALP report.

We continue to believe that the programs at Peach Bottom are fundamentally sound

and basically acceptable. However, an assessment of our inspection findings

indicates that there are certain underlying factors that inhibit improvements

in the overall performance of Peach Bottom. One such factor is your dependence

on third parties to identify problems rather than implementing proactive internal

methods to assess site program effectiveness, self-identify weaknesses and

implement effective long-range actions. Another factor is an apparent attitude

that identified weaknesses will somehow be resolved by corrective action plans

without aggressive followup and oversight by managers. A related factor is an

- apparent tendency to target accountability for corrective action programs at

too low a level to assure effective implementation. The cause of these factors

must, in our view, be addressed and appropriate changes made in order to

achieve the necessary improvements.

We also identified one violation of NRC requirements as discussed in the

enclosed Notice of Violation. This finding is particularly significant in that

it results from ineffective short term corrective actions resulting from a

program implementation weakness previously identified to you. In your response

to the attached Notice of Violation, please also address the items identified

as licensee response items in Attachment 2 of the enclosed report.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.

After you have reviewed this report, please call me and I will arrange a

meeting in order to examine the results of this inspection in more detail.
