Box 195 Wayne, Pennsylvania 19087-0195 SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-277/98-11, 50-278/98-11, 72- 1027/98-11 AND NOTICE OF VIOLATION ..- Dear Mr. Rainey: On January 4,1998, the NRC completed an inspection at the Peach Bottom Atomic Power ' Station. The enclosed report presents the results of that inspection. ( Your staff continued to operate both units safely. Control room personnel performer 1 wet during - two separate electrical transients and during the Unit 3 load changes to repair an instrument _ nitrogen leak and replace the main turbine lubricating oil baffier valve and pump. During the inspection period, we noted that an inadequate breaker manipulation procedure resulted in the unexpected loss of one off site power source. Subsequent operator action was required to restore affected plant equipment to a normal plant lineup. Previous procedure adequacy problems were documented in inspection Reports 50-277(278)/98-02 and 50- 277(278)/98-06. Your staff's continued attention to this problem is warranted. . Based on the results of this inspection and an NRC investigation, the NRC has determined that one violation of NRC requirements occurred. The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding it are described in detail in - the subject inspection report. The violation is of concern because: 1) three licensed F operators failed to complete your facility licensed operator requalification program for the period April 1994 through March 1996 and the training was not made up until April 1998, in some cases; 2) the failure was due to a program inadequacy (systemic cause) and the inadequacy apparently caused an inaccurate license renewal application to be submitted to the NRC upon which the NRC issued a renewed operator license. Our investigation found that the violation was not willful but was related to programmatic causes. The 01 , investigation synopsis is attached to the enclosed inspect;on report. / - The NRC concluded that your review of this matter was thorough after its identification in ' February 1998. You also identified that the lack of a single point of contact in your process for _ preparing and sending the forms was another cause of the inaccurate license renewal f application.
