11. LaSalle County Station: Unit 1 111. Event Description On 5/14/82, excessive amounts of water was flowing through the north run-off ditch, and into the Cooling Pond. Through investigation it was determined that the source of the water was a vacuum breaker standpipe for the Vastewater Treatment Lift Station Discharge Line OTW89B. Untreated and unmonitored wastewater was being discharged from this standpipe, and flowing into the run-off ditch. This condi-tion is a violation of the Station's NPDES Permit No. ILOO48151.

IV. Probable Consequences of the Occurrence This occurrence did not affect the health and safety of the general public.

V. Cause Discharge Line OTW89B vacuum breaker standpipe does not contain a

• check valve to prevent the inadvertent spillage of wastewater during normal operation.

VI. Corrective Action The initial interim corrective action was to plug the standpipe to prevent leakage. Th'is was first done on 5/5/82. Due to the pressures

""~ developed under normal operation, the plug kept coming out. A per-manent corrective action to install a new check valve was initiated through Work Request No. 15398. -
