The temporary hydrogen peroxide system was not injecting hydrogen peroxide.

CORRECTIVE ACTION _:

A work request was written to repair the malfunctioning pump.

Repairs to the other injection pump in the system had been completed. This pump was put in service, and verification made that it was injecting hydrogen peroxide.
