Environmental Protection Plan Operating Report Appendix B to i Facility License No. NPF-11 and NPF-18 l Attached is the Annual Environmental Operating Report for 1996 for the Environmental Protection Plan as required by Section 5.4 of Appendix B to the Facility License No. NPF-11 and NPF-18.

This report includes information required by the following subsections of Appendix B:

3.1 Plant Design and Operation 4.2.1 Vegetative Integrity on Cooling Pond Dike 5.4.1 EPP Noncompliances and the Corrective Actions to Remedy Them ,

No changes in station design or operation, tests, or experiments which can be classified as a potentially significant unreviewed environmental issue (per Subsection 3.1) were made.

Venetative Intearity on Coolina Pond Dike The vegetative integrity on the cooling pond dike was inspected in accordance with LaSalle County Station Surveillance LTS-100-5. Inspections during 1996 were completed on April 17, May 17, June 20, July 24, August 27, l September 25, October 24 and November 25. The results of the inspections were as follows:

. The vegetative growth on the LaSalle Lake dike was in generally good i condition. At the beginning of the growing season there was a bare area l at the downstream toe between markers 100 and 180 where clearing of trees was performed the previous fall. This area did not require I

reseeding. Also, on top of the dike, along the uad, there was a small eroded area at marker 141 and a bare area ; narker 118 where the buried fiber optics cable was repaired. The bare area at marker 118 was reseeded and by the end of the growing season this area had filled in well.

. The Canadian Thistle infestation problem was evaluated and at present no acceptable method to remove the thistle has been found. The ground cover used on the dike to prevent erosion is Crown Vetch. The acceptable sprays that would remove the thistle would also eliminate the Crown Vetch. No further actions are being considered at this time. ]

There were no other vegetative concerns noted during the 1996 cooling pond j dike inspections. l E_PP Noncompilances and Corrective Actions Taken to Remedy Them During 1996, LaSalle County Station experienced one noncompliance condition.

This event was submitted to the Illinois Environmental Protection Agency (IEPA) in accordance with Standard Condition 12 of the station's NPDES Permit No. IL0048151. The following contains a description of the noncompliance and corrective actions taken:

I On Thursday, August 8,1996, during normal plant operations, the Cooling  !

Pond Make Up Pump "A" was started at 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br />. At approximately 1144 hours0.0132 days <br />0.318 hours <br />0.00189 weeks <br />4.35292e-4 months <br /> the "A" pump was secured due to low discharge pressure and no water flow from the Make Up Structure. Two Operators were 1 dispatched to inspect the cooling Pond Blowdown and Make Up lines. l At 1233 hours0.0143 days <br />0.343 hours <br />0.00204 weeks <br />4.691565e-4 months <br />, a Cooling Pond Make Up line rupture was confirmed. At this time, the "C" pump was secured. Consequently, the ruptured line caused an unpermitted discharge of Cooling Pond Make Up water to the Illinois River.

A calculated six million gallons of water was discharged to the river based on the Lake Make Up pump discharge flow for two hours, three minutes duration.

As a precaution, the Cooling Pond Blowdown line was isolated on August 8,1996, at 1516 hours0.0175 days <br />0.421 hours <br />0.00251 weeks <br />5.76838e-4 months <br />. The Chemistry Department collected a sample of the water discharged. Sample analysis resulted in a pH of 7.78 S.U.

The plant Site Engineering Staff and Mechanical Maintenance Department inspected the ruptured line and initiated repairs on Friday, August 9,1996. Repairs were completed on August 22,1996.
