Gentlemen:

Subject: Inspection Report No. 50-293/86-25

This letter refers to the routine safety inspection (50-293/86-25) conducted by

Dr. M. McBride and others of this office on July 8,1986 to August 4,1986 at

the Pilgrim Nuclear Power Station, Plymouth, Massachusetts. The findings from

the inspection were discussed by Dr. M. McBride with Mr. A. .Pedersen and other

members of your staff periodically during the inspection and at the conclusion

of the inspection.

Areas examined during this inspection are described in the NRC Region I Inspec-

tion Report which is enclosed with this letter. Within these areas, the in-

spection consisted of selective examinations of procedures and representative

records, interviews with personnel, and observations by the inspector.

Based on the results of this inspection, it appears that some of your

activities related to corrective action and procedural adherence were not

conducted in full compliance with NRC requirements, as set forth in the Notice

of Violation, enclosed herewith as Appendix A. These violations have been

categorized by severity level in accordance with the revised NRC Enforcement

Policy (10 CFR 2, Appendix C) published in the Federal Register Notice (49 FR

8583) dated March 8, 1984. In accordance with NRC regulations 10 CFR 2.201

you are required to respond to this letter within 30 days of its receipt.

In addition, it appears that your fire brigade training program does nit fully

implement commitments contained in your submittals to the NRC, as set forth in

the Notice of Deviation, enclosed herewith as Appendix B. You were requested

to respond to this concern in inspection report 86-21. If this response ad-

dresses the areas listed in Appendix B to this report, a second response is

not required.

The responses directed by this letter and the accompanying Notice are not

subiect to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
