PILGRIM INTEGRATED INSPECTION REPORT NO. 50-293/99-04

Dear Mr. Bellamy

This refers to the inspection conducted on June 10,1999, through July 25,1999, at the Pilgrim

facility. The enclosed report presents the results of this inspection. During this inspection

'

period, your conduct of activities at the Pilgrim facility during RFO12 and during power

ascension activities were generally characterized by safety conscious operations, sound

engineering and maintenance practices, and careful radiological controls. Several material

condition upgrades were made to the secondary plant during the refueling outage to improve

overall plant reliability and efficiency, including the installation of newly designed feedwater

regulating valves and 18 new steam plant dump and centrol valves. We did note a problem

related to human performance (configuration control) when operators inadvertently drained ten

inches of water from the reactor vessel to the torus while aligning the residual heat removal

system for automatic initiation. Considering the previous problems in this area (tagout

implementation problems during the outage), we conclude that more management attention is

needed to address this concem.

Some maintenance induced quality problems on the main steam iso!ation valves (MSIVs) arose

during the latter part of the refueling outage which required repeat maintenance work and

additional radiation worker exposure. Each problem was corrected before power ascension

,

commenced; however, we believe there were some underlying issues that merit your attention.

For example, the oversight and experience level of contract workers was a contributing factor.

We were informed that your outage management staff will perform a lessons teamed review to

'

capture issues like this to improve future performance. Additionally, we also understand that

your quality assurance organization commenced a vertical slice review of all outage

maintenance rework issues to identify and correct any common causes. Lastly, the operators

retumed the unit to power in a conservative manner and effectively mitigated the effect of a

water hammer event when placing the feed water system regulating valves into service.

Based on the results of this inspection, the NRC has determined that two (2) level 4 violations of

NRC requirements occurred. These violations are being treated as Non-Cited Violations

(NCVs), consistent with Appendix C of the Enforcement Policy. These NCVs involved: (1) an

operator procedure use issue resulting in the lowering reactor vessel level and (2) an

inadequate safety evaluation for EDG TMOD. These NCVs are further described in the subject

inspection report. If you contest the violation or severity level of these NCVs , you should

provide a response within 30 days of the date of this inspection report, with the basis for your

denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington DC

20555-0001, with copies to the Regional Administrator, Region 1, and the Director, Office of

Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.
