' Subject: NRC Region I' Special Inspection (50-293/90-21) of the Plant Shutdown

that occurred on September 2, 1990, at the Pilgrim Nuclear Power

Station,

This letter refers to.the_Special Inspec Uon conducted by Mr. N. Conicella

on September 5-7, 1990, at the Pilgrim huclear Power Station, Plymouth,

Massachusetts. The inspection was conducted to review the event of

' September 2-3 which involved a number of component malfunctions;and operational ,

complications following shutdown. At the conclusion of the inspection, an exit J

interview was held with members of your staff to discuss the inspection findings. ,

,

Areas examined during the inspection are described in the Region I Inspection i

Report which is onclosed with this letter. l

'The inspection team concluded that the licensed operators on shift at the time

of the' plant shutdown performed appropriately in mitigating the effect of the

component failures and placing the reactor in a safe condition. The-inspection

~

team also' concluded > that the, Multi-Disciplined Analysis Team (MDAT) proved to

be an effective team for determining all the problems that occurred, the root

causes for the problems, and the appropriate corrective actions.

The NRC is concerned that several of the component malfunctions that occurred

were directly related to less .than adequate maintenance practices. Additionally,

during the conduct of.the inspection, the inspectors noticed poor work practices ,

by maintenance personnel. You should focus your attention to Section 4.0 of I

the report for the details'of the maintenance problems noted by-the NRC

inspection team. The' NRC understands that you- have previously identified j

maintenance problems through self-assessments and that you have instituted.

programs to correct these problems. In future inspections, the NRC will

monitor your- progress in resolving these maintenance issues. I
