NRC INTEGRATED INSPECTION REPORT NO. 50-293/99-01
Dear Mr. Sullivan:

This refers to the inspection conducted on January 25,1999 through March 7,1999, at the
Pilgrim Nuclear Power Station in Plymouth, Massachusetts. The enclosed report presents the
results of this inspection.
. During the six weeks covered by this inspection period, the conduct of activities at the Pilgrim
facility was characterized by safe plant operations. This was especially evident during planned
evolutions such as during new reactor fuel inspections and also during a planned down power
. outage at 50% reactor power. In both instances, good planning of the activities were observed,
and your staff performed good mock-up training and oversight to promote positive human
performance.
Based on the results of this inspection, the NRC has determined that two violations of NRC
requirements occurred. These violations are being treated as Non Cited Violations (NCVs),
consistent with Appendix C of the Enforcement Policy. These NCVs involved: (1) human error
leading to a valve misalignment in the stator cooling water system, and (2) a design control
error affecting the control room air filtration system that was reported in LER 50-293/98-24.
These NCVs are further described in the subject inspection report. If you contest the violation
or severity level of these NCVs , you should provide a response within 30 days of the date of
this inspection report, with the basis for your denial, to the Nuclear Regulatory Commission,

In the area of radiological controls, we reviewed several aspects of your program. Your limited
site radiological characterization revealed small amounts of radioactive contamination present
in a few onsite soil samples. We found that your staff maintained appropriate records of spills

in accordance with 10 CFR 50.75 (g) and took adequate corrective actions to evaluate and

document the soil samples. Radiological planning was properly focused on maintaining
l
radiation dose for refuel outage (RFO) 12 as low as is reasonably achievable (ALARA). The
security program was also inspected during this period. No safety concerns or violations were
identified. '
