This letter approves reactor operation up to seventy-five (75) percent of full

power in accordance with Boston Edison Company's (BECo) Power Ascension Test

Program and NRC Confirmatory Action Letter 86-10 and its supplements. This

letter neither imposes any new requirements nor releases Boston Edison from any

existing requirements contained in the Facility License or Technical Specifi-

. cations.

The staff finds that management performance, plant material condition

and operational performance support proceeding to the next phase of the Power

Ascensior Program.

.

The bases for approving the continuation of the Power Ascension Program consist

of:

(1) successful completion of the Power Ascension Program up to 50% power,

(2) a comprehensive assessment of plant and personnel performance by BECo dur-

ing this phase of plant startup, and (3) NRC staff inspection and review acti-

vities which have confirmed the safety of operations by both the staff and

management of BECo during this phase of plant startup.

Enclosure (1) is a sum-

mary of NRC findings which either have been or will be docketed in NRC inspec-

tion reports 50-293/89-07 and 89-08. The most recent inspection activities, by

the NRC Restart Staff, monitored and verified selected aspects of your power

ascension activities including your own management assessments of the perform-

ance of your facility and personnel.

Based upon our conclusion that the facil-

ity is being safely operated and satisfitctorily managed, and is ready for the

next phase of power ascension, the NRC has no objection to plant operation at

!

power levels up to 75% of full power.

Additionally, the current status of emergency preparedness for the communities

i

surrounding the Pilgrim Station as compiled by the staff and reported to the

!

Commission is shown in Enclosure 2.

We continue to encourage cooperative and

expeditious action by Boston Edison and the Commonwealth and local governments

!

to further improve the state of emergency preparedness for the area surrounding

!

the Pilgrim facility.

Assessment of Licensee Readiness for Release

From NRC Approval Point Number 4 Permitting Operation to 75% Power

I.

Background

NRC Confirmatory Action' Letter No. 86-10 of April 12, 1986,- and the

August 27, 1986, supplemental letter. identified specific technical issues

to be resolved prior to facility restart.

In addition, due'to other long-

term management, hardware and program issues, Boston Edison Company (BEco)-

submitted a formal power ascension program to the NRC. As specified in

the licensee's NRC-accepted Power Ascension Test Program, the licensee

must obtain the NRC Region I Regional Administrator's agreement to proceed

beyond the 50% NRC approval point to the 75% power. plateau. The program

includes NRC Regional Administrator approval points at criticality, 5%,

I

25%, 50%, 75% of full power, as well as a licensee formal assessment and

NRC review after completion of the Power Ascension Program, including

testing at full power.

By letter dated June 26, 1989, Mr. William T. Russell, Regional Adminis-

trator for Region I, agreed with BEco's conclusion to proceed beyond the

third approval point and conduct power operations up to 50% of full power.

This enclosure provides the NRC Restart Assessment Panel's assessment of

licensee performance since the licensee's request for release from the

' third approval point.

III. Staff Evaluation

1.

The NRC Restart Assessment Panel considered the following approval

point release criteria to evaluate the conduct of the Pilgrim Power

Ascension Test Program and consider approval of the licensee's con-

tinuation of power ascension up to 75% of full power:

a.

Plant status and equipment operability

b.

Management / personnel performance and oversight activities

c.

Dower Ascension Testing Program adequacy

d.

Power Ascension Test Program results

e.

NRC inspection coverage and program status

2.

Staff assessment of licensee performance is as follows:

a.

Plant Status and Equipment Operability

The licensee has determined and the NRC is satisfied that all

equipment required for continuation of the Power Ascension Pro-

gram is operable. When equipment inoperabilities have occurred,

l

the licensee has handled them conservatively and within Tech-

I

nical Specification requirements.

Required non-safety-related

equipment is in a condition that supports safe plant operation.

,

I

Surveillance tests are up-to-date with no significant identified

discrepancies. The licensee's management review of facility

operation up to fifty percent power is complete. Operation of

the facility was conducted in a safe manner and concerns raised

by the NRC and BECo management regarding facility operations

have been appropriately addressed.

During this power ascension plateau, the licensee has experi-

enced degraded condenser vacuum due to fouling of the condenser

tubes with mussels and algae.

Following the manual reactor scram on July 18, 1989, the licensee performed manual cleaning

of the condenser water boxes, the salt service water bay and the

turbine building and reactor building component cooling water

l

heat exchangers in order to remove the buildup of mussels and

l

algae. The Pilgrim plant restarted on July 26, 1989, following

'

minor maintenance and removal of accessible fouling. The licen-

see then returned the plant to the 50% power level plateau and

completed core performance surveillance testing.

On August 2, 1989, following removal of the "C" condensate pump

suction strainer, the pump and strainer box were being filled

from the discharge of the "A" condensate pump prior to placing

the "C" pump back into service. The pump strainer box and ex-

pansion joint sustained structural deformation due to overpres-

surization. The "C" condensate pump and related equipment is

not safety-related and does not impact operation at 50% power.

The condensate pumps are described in the FSAR as one-third

<

,

capacity, and normally all the condensate pumps are used above

60% power. tFollowing the event, the licensee performed evalu-

.

attons that concluded that the plant could operate at 75% with

"

two.of three condensate pumps operating. Repairs were completed-

and tested August 14 to restore operability of the third pump.

Licensee review determined that the root cause was that the pro-

cedure intended to govern the particular fill and vent operation

!

was not accuretely followed and this deviation from procedural

adherence resulted in the structural damage to the pump streiner

box and expansion joint. The licensee has fabricated and in-

-

stalled a new spool piece with a vent line in place of the dam-

aged strainer box and has replaced the expansion joint. De-

tailed engineering analyses on affected condensate piping and

components have shown that the condensate system is acceptable

for full power operation.

The NRC Restart Assessment Panel is satisfied that all equipment

required for continuation of the Power Ascension Program is

operable.

b.

Management / Personnel Performance and Oversight Activities

The NRC Restart Staff has continued to monitor management and

personnel performance on an as-needed basis, including periods

of around-the-clock coverage since startup. BECo management has

been found to be responsive to NRC concerns and to be performing

in a cautious, deliberate manner.

Licensee peer evaluators were

observed and have been providing good feedback.to personnel.

Operators were generally knowledgeable of their duties and plant

conditions and management maintains an active and effective

oversight of operations.

The Restart Inspection Staff observed Phase I of the licensee's

SDOCR test. The licensee's preparation, conduct and critique of

the SDOCR test was judged to be thorough, professional and com-

plete. -Command and control of all evolutions outside the con-

trol room by the test crew Nuclear Watch Engineer was excellent

and communications between test crew personnel were formal and

complete. The NRC roncluded that the overall test was highly

successful. Phase 2 of the test will be completed in conjunc-

tion with scheduled outage shutdown in the fall of 1989.

As 'a result of the decreasing condenser vacuum experienced dur-

ing the swap over of steam jet air ejectors on July 18, 1989,

=the licensee manually scramtred the. reactor prior to reaching the

automatic reactor scram setpoint for low condenser vacuum. This

. action by the licensee was prompt and conservative.' The licen .

see also aggressively investigated the cause of the air ejector

failure and implemented appropriate corrective actions.

As a result of the structural damage in the "C" condensate pump

strainer box and expansion joint, licensee management pursued

,

i.

and addressed the root causes of the overpressurization. The

l

licensee has reviewed and assessed their published dite-tives

'

and policies regarding procedural compliance and conducted 3c

terviews with all'11 censed operators to determine their under-

standing of procedural compliance for balance of plant as well

as safety systems. Appropriate corrective- actions have been

implemented.

Operators have'been briefed on management's policy

regarding procedural compliance. An evaluation of balance of

plant activities which currently are not procedurally controlled

is being conducted to determine the appropriateness of having a

procedure to control the activity. The Restart Panel assessed

this event to determine if the causes wete related to those

identified during the April 1989 AIT or the June 9, 1989 RCIC

valve area temperature monitoring event. The Ptnei concluded

that the causes of the August 2, 1989 event were not so closely

related to those of the eaclier events as to indicate ineffec-

tiveness of prior corrective actions.

Licensee actions in re-

sponse to each event have been reasonably aggressive and

thorough.

The NRC Restart Assessment Panel has concluded that the licensee

management and plant staff performance support proceeding to 75

percent of full power.

c.

Power Ascension Testing Program Adequacy

The licensee's Power Ascension Test Program continues to be in

place and implemented in accordance with the licensee's commit-

ments documented in the December 30, 1988 supplement to Confir-

matory Action Letter 86-10. No significant problems or tech-

nical issues are outstanding as a result of implementing this

program.

Power Ascension Test Program Results

The NRC is satisfied that tests and conditions required to be

completed by the licensee during performance of the NRC-accepted

Power Ascension Program up to 50 percent of rated power have

been completed. Satisfactory results were achieved during the

performance of:

Also, the NRC evaluated licensee management and plant staff per-

formance during planned periods of normal operations at about

l

50% of rated power. During this period, licensee management

'

had, as planned, reduced the level of oversight of operations.

The NRC panel has concluded that plant operations are being con-

ducted safely and conservatively.

The NRC has reviewed BEco's actions on those inspection findings

which had potential to impact further reactor operations and

found no outstanding issues which would preclude the licensee

fro:n proceeding up to 75% full power.

e.

NRC Inspection Coverace and Program Status

The NRC integrated inspection plan covering activities from 50%

to 75% rated power has been approved. The inspection plan and

inspection coverage levels are equivalent to those delineated in

the Pilgrim Restart Staff Operating Plan dated October 13, 1988,

for the Power Ascension Test Program.

There are no open allegations at Pilgrim requiring fuct.her in-

spection.

There are no enforcement actions pending which would precluoc

continued plant operations. Although deliberations are ongoing

to estabiish the appropriate enforcement actions for the

April 12, 1989 RCIC system overpressurization event, the licen-

see has already implemented corrective actions.

Reviews by the

NRC Staff indicate that the corrective actions for these events

were acceptable and formed a reasonable basis to conclude that

such events would not recur. Therefore, the panel concludes

that any pending enforcement decision for the April 12, 1989,

event does not preclude operat un up to 75% of full power.

IV.

Conclusion

The NRC continues to r- itor the facility operation and licensee actions

in conjunction with ...e

Pilgrim Nuclear Power Station Power Ascension

Program. The licensee has completed the activities of the Power Ascension

Program from 25% to 50% power. BECo has conducted oversight of those ac-

tivities, assessed performance and requested agreement from NRC to proceed

to the next NRC approval point of 75% power. The NRC Power Ascension Pro-

gram Restart Inspection Staff has monitored licensee activities and con-

ducted an independent assessment of operational training, personnel and

equipment performance. The NRC Restart Panel noted that the licensee ap-

proached plant operations in a conservative, safety-conscious manner.

On July 19,1989 (and subsequent telephone conversations on July 24,

'

1989, and August 7, 10 and 14, 1989), the Pilgrim Restart Assessment Panel

convened in the Region 1 Office to review licensee readiness to proceed

beyond 50% power. Presentations were made by the licensee and the NRC Re-

start Manager. The basis for the BEco performance evaluation conclusion

was reviewed and NRC inspection activity results were assessed. Based on

these activities, the Restart Assessment Panel is satisfied that plant

operations up to 75% of full power can proceed in a safe and deliberate

manner.

Current Status of Emergency Preparedness for the

Communities Surrounding the Pilgrim Station

Progress continues to be noted in the state of emerge :ty preparedness for

Ptigrim in the areas of training of personnel, in the identification and reso-

lution of Reception Center issues, in supplying equipment to local towns, in

the active and visible involvement in the review process by the Federal Emerg-

ency Management Agency (FEMA), and in the scheduling and planning for the

October 1989 full participation emergency exercise.

Plymouth has 91 of. 'R implementing procedures approved in concept by the Beard

of Selectmen. All 92 procedures have been forwarded to the Massachusetts Civil

Defense Agency (MCDA) for review and Plymouth has authorized MCDA to forward

all but the school procedure to FEMA. Plymauth has requested technical assist-

ante from MCDA on the school procedure.

In Duxbury, all 44 of .the procedures have been reviewed by the Department Heads

and the review committee and were forwarded to the Board of Selectmen on

May 1, 1989. The Board of Selectmen met on May 12, 1989, to review the proced-

ures. FEMA attended this meeting. The procedures were returned to the review

committee for further review and comment. Additionally, Department Heads were

tasked with responding to specific Selectmen questions.

The other towns have received a copy of the FEMA technical review of their

emergency plan'and implementing procedures. There are no major issues identi-

fied and the towns are working to address FEMA comments.

Overall training continues to progress. The Commonwealth has been working

closely with BECo and the towns to approve lesson plans. To date, 15417 of

'0397 hours0.00459 days <br />0.11 hours <br />6.564153e-4 weeks <br />1.510585e-4 months <br /> of training for state, federal and local agencies has been com-

-leted.

Progress continues on the reception centers.

In Taunton, all equipment is in

place and available for use. Staff has been identified for all reception cen-

ter activities and training is continuing.

In Bridgewater, r.1.1 equipment is in

the town and available for use at the reception center. Staff has been identi-

fied'for first shift operations and approximately 50% of second shift opera-

tions. The Bridgewater State College Board of Trustees approved the use of the

college as a reception center. The Chairman has been empowered to enter into a

Letter of Agreement for this use. This agreement has been reviewed by legal

counsel, executed by BECo and forwarded on May 25, 1989, to the Board of Trus-

tees.

In Wellesley, decontamination facilities have been located on-site.

There are no major equipment needs outstanding.

BECo and the Commonwealth are

actively working on identifying Congregate Care tacilities as well as addi-

tional parking arrangements at Wellesley. Training for Department of Public

l

Works personnel has commenced.

The Commonwealth has agreed to the publishing of the Public Information Bro-

chure (PIB). The PIB was sent to the publisher on July 10, 1989.

It is sched-

uled to be distributed in late August or edrly September, 1989.

The staff has continued to attend planning seetings with town Civil Defense

Directors, local elected officials, Commonwealth and MCDA Area II of ficials

that discuss the status of off-site emergency preparedness.

Notable among

these meetings were meetings held on June 19, 1989 and July 17, 1989, between

FEMA and the Commonwealth Secretary of Public Safety. At the meetings, the

Secretary stated that the Commonwealth would continue to work closely with BECo

and FEMA to resolve the remaining planning issues and, in particular, continue

preparation for the upcoming October 1989 full participation exercise. At the

July 17, 1989 meeting, Secretary Barry discussed the agreements that had been

negotiated with BECo regarding the remaining planning issues.

FEMA continues

to be actively involved with the Commonwealth ano local towns, attending many

meetings and work sessions, as well as the regular weekly planning meetings.

Significant meetings scheduled include a July 31, 1989, follow-up meeting be-

tween FEMA and the Commonwealth Secretary of Public Safety.

The attached emergency preparedness summaries provide the July 20, 1989 status

of selected emergency preparedness issues for each town surrounding the Pilgrim

Station, the Commonwealth and MCDA Area II as compared to that of June 2,1989.

The staff provided the summaries to the respective town officials through the

Civil Defense Directors of each area and subsequently contacted each official

to verify the accuracy of the summaries.
