50-328/87-57) Areas Inspected: Announced inspection to complete the review of TVA's implementation of a program per the requirements of 10 CFR 50.49 for establishing and maintaining the_ qualification of electric equipment within the scope of 10 CFR 50.4 Results: The inspection identified no deficiencies in the SQN implementation cf a i o program to meet the requirements of 10 CFR 50.49. Based on the inspectors reviews and evaluations. Inspection Follow-up Items 50-327,326/87-22-02 and -03 were closed. While no deficiencies were identified in Equipment Qualification program implementation, SQN needs to complete the Replacement Items Project (RIP) before SON is in compliance with 10 CFR 50.49. This lq project.is the subject of a separate NRC inspection (inspection 50-327,328/87-40, July 13-24, 1987); therefore, completion of RIP is not identified for follow-up in this inspection report

On August 21-22, 1985 TVA shut down both units at SON due to concerns that documentation at TVA nuclear sites might be inadequate for environmental qualification of electric equipment within the scope of 10 CFR 50.49. Since that time, TVA has initiated an in-depth program to ensure that environmental qualification of elert.ric equipment within the scope of 10 CFR 50.49 is established at SON prior to restart (Note: certain Regulatory Guide 1.97 equipment has been exempted for restart due to a later implementation date accepted by the NRC), In order to evaluate the new program at SON, the NRC has conducted five inspections of the SQN program and has documented the results in NRC Inspection 1 Reports 50-327,328/86-01, dated August 15, 1986, 50-327.328/86-70, dated March 9, 1987, and 50-327,328/87-22, dated June 17, 1987. This inspection is ,

   -2-a continuation of the NRC evaluation of the SON equipment qualification (EQ)

program and its implementation. This inspection focused en the activities completed since the last EQ inspection (April 6-10,1987) and included review of TVA's actions on the inspection follow-up items identified in the last EQ inspection repor No new items were identified as needing completion during this inspectio . FINDINGS A: Follow-up of previous inspection Findines The NRC inspector reviewed SQN's actions relative to two inspection follow-up items identified during the last EQ inspection (April 6-10, 1987). Status of the items is discussed in the following paragraph (1) (Closed) Qualification Requirements for Certain Events (50-327,328/87-57-02) This item pertained to the TVA determination that for a main-steam-line break (MSLB) inside containment, certain localized temperatures (pressurizer and lower compartment) for the time period from one half a day and beyond after event initiation were not enveloped by existing qualification profiles. The maximum deviation from the qualification profiles was 17 degrees Fahrenhei In order to correct this problem, TVA chose to qualify the contain-ment coolers (including the air return fans); thereby, mitigating the effects of the event. In order to qualify the coolers, TVA rewound the f an motors to a qualified configuration and documented the qualification in new qualification binder, SONEQ-MOT-00 Additionally, TVA revised binders SONEQ-SPLC-001 and SONEQ-CABL-009, to document qualification of splices and cables used in conjunction , with the qualified motors. In addition to qualifying the coolers, TVA disabled the temperature control loop of the throttling valves controlling essential raw cooling water (ERCW) flow to the coolers in order to run the valves in either a closed or full open position as required by plant conditions. This change in the temperature control loop required " Tech Spec" ( C) change The inspector reviewed the new motor binder and revised splice and cable binders; as well as, proposed TS changes, field verification , sheets, design change notices, engineering change notices, and other available documentation supporting the activities necessary to resolved this item. The inspector determined that the motor binder was not formally issued; however, it was through management review and did support qualification of the motors. The inspector's review determined that formal issuance of the binder and completion of two field activities (drilling weep holes in the air return motor junction boxes and installation of motor name plates) had been identified by TVA for completion prior to full resolution of this l i - _-

  '(2) (Closed) Completion of EQ Fieldwork This item pertained to the completion of 13 Unit 2 E0 fieldwork open items icentified in TVA's letter to Mr. S. Ebneter, NRC, from R. Gridley, dated March 24, 1987. The inspector reviewed 18 qualifica-tion binders, 23 environmental qualification information releasas (EQIRs), 3 quality information releases (QIRs), 2 SAL closure forms, 1 engineering change notice (ECN), and 1 internal TVA letter relative to the completion of the 13 items. The inspector determined that the items had been completed except for installation of acoustic monitor electrical splices, EQ Project verification of installation of all Raychem splices (splices were physically installed but had not been checked against splice lists in qualifica-tion binders), coating of terminal blocks in the valve vaults following functional testing, changing of setpoints for four devices due to accuracy calculations. During the inspection, the inspector reviewed two QIRs which indicated that changing of setpoints for the four devices would not be required; however, resolution of this was not finalize Since the inspector was able to verify completion of the majority of these fieldwork open items and the remaining items are being tracked for completion on the "Sequoyah Equipment Qualification Open Items List" and/or the " SON Startup Activities List (SAL), this inspection follow-up item is close Even though the inspector reviewed the completion of a majority of these items, TVA needs to formally notify the NRC when all Unit 2 EG fieldwork is completed (Unit I notification is also required). This notification is the last remaining confirmatory item identified in the draft SON EQ safety evaluation transmitted to TVA on December 5, 198 Plant Physical Inspection The NRC inspector physically inspected the newly installed lower containment cooler air return fan motor, 2-MTR-30-75, and did a walkdown inspection of the Unit 2 West valve vault room to examine the inplant activities completed by SON in resolving the valve vault hot spot issu The inspector examined the motor and associated cooler for characteristics such as mounting configuration, orientation, interfaces, model number, ambient environment, and physical condition. The inspector examined the valve vault for changes (addition of insulation to piping and conduits and

The inspection focused on activities completed since the last EQ inspection conducted on April 6-10, 1987. Within these areas, the inspection consisted of examination of selected procedures and records, interviews with personnel and observations by the inspecto This inspection identified no deficiencies in the SON implementation of a program to meet the requirements of 10 CFR 50.49. Based on the inspectors' '3 reviews and evaluations, Inspection Follow-up Items 50-327,328/87-22-02 and -03 were closed. Details of the inspection are discussed in the enclosed inspection repor While no deficiencies were identified and the follow-up items were closed. SON still has to complete a few activities, including the Replacement Item Project (RIP), before compliance with 10 CFR 50.49 is attained. Since RIP is the subject of separate NRC inspection activities, it is not addressed in this report. With regard to fieldwork, TVA needs to formally notify the MC when l Unit 2 EQ fieldwork is completed (Unit I notification is also required). This notification is the last remaining confirmatory item identified in the draft SON EQ safety evaluation transmitted to TVA on December 5, 1986.

l j BACKGROUND

On August 21-22, 1985, TVA shut down both units at SQN due to concerns that { documentation at TVA nuclear sites might be inadequate for environmental l qualification of electric equipment within the scope of 10 CFR 50.49. Since j that time, TVA has initiated an in-depth program to ensure that environmental I qualification of electric equipment within the scope of 10 CFR 50.49 is , I established at SQN prior to restart (Note: certain Regulatory Guide 1.97 equipment has been exempted for restart due to a later implementation date accepted by the NRC).

! In order to evaluate the new program at SON, the NRC has conducted five inspections of the SQN program and has occumented the results in NRC Inspection i l Reports 50-327,328/86-01, dated August 15, 1986, 50-327,328/86-70, dated March 9, 1987, and 50-327,328/87-22, dated June 17, 1987. This inspection is

l t-2-i a continuation of the NRC evaluation of the SQN equipment qualification (EO) program and its implementation. This inspection focused en the activities completed since the last EQ inspection (April 6-10,1987) and included review ! of TVA's actions on the inspection follow-up items identified in the last EQ inspection report, , No new items were identified as needing completion during this inspectio . FINDINGS i A: Follow-up of previous Inspection Findines The NRC inspector reviewed SON's actions relative to two inspection . follow-up items identified during the last EQ inspection (April 6-10, l 1987). Status of the items is discussed in the following paragraph (1) (Closed) Qualification Requirements for Certain Events  ;

This item pertained to the TVA determination that for a main-steam-line break (MSLB) inside containment, certain localized temperatures (pressurizer and lower compartment) for the time period from one half a day and beyond after event initiation were not enveloped by existing qualification profiles. The maxinum deviation from the qualiff cation profiles was 17 degrees Fahrenhei l

In order to correct this problem, TVA chose to qualify the contain-ment coolers (including the air return fans); thereby, mitigating the effects of the event. In order to qualify the coolers, TVA rewound the fan motors to a qualified configuration and documented ! the qualification in new qualification binder, SONEQ-MOT-00 Additionally, TVA revised binders SONE 0-SpLC-001 and SONEQ-CABL-009, to document qualification of splices and cables used in conjunction with the qualified motor In addition to qualifying the coolers, TVA disabled the temperature control loop of the throttling valves controlling essential raw cooling water (ERCW) flow to the coolers in order to run the valves in either a closed or full open position as required by plant conditions. This change in the temperature control l loop required " Tech Spec (TS) change The inspector revieweo the new motor binder and revised splice and cable binders; as well as, proposed TS changes, field verification ! I sheets, design change notices, engineering change notices, and other I available documentation supporting the activities necessary to resolved this item. The inspector determined that the motor binder l was not formally issued; however, it was through management review and did support qualification of the motnrs. The inspector's review determined that formal issuance of the binder and completion of two l field activities (drilling weep holes in the air return motor i junction boxes and installation of motor name plates) had been j identified by TVA for completion prior to full resolution of this l

Since the inspector considered l the. remaining activities to be minor and the activities were being tracked for resolution on the "Sequoyah Equipment Qualification Open Items List" and/or the " SON Startup Activities List (SAL), this inspection follow-up item is close (2) (Closed) Completion of EQ Fieldwork This item pertained to the ccmpletion of 13 Unit 2 EQ fieldwork open { items identified in TVA's letter to Mr. S. Ebneter, NRC, from R. ' Gridley, dated March 24, 1987. The inspector reviewed 18 qualifica-tion binders, 23 environmental qualification information releases (EQIRs), 3 quality information releases (QIRs), 2 SAL closure fortus, i 1 engineering change notice (ECN), and 1 internal TVA letter relative to the completion of the 13 items. The inspector determined that the items had been completed except for installation , of acoustic monitor electrical splices, EQ Project verification of installation of all Raychem splices (splices were physically ' installed but had not been checked against splice lists in qualifica-tion binders), coating of terminal blocks in the valve vaults following functional testing, changing of setpoints for four devices due to accuracy calculations. During the inspection, the inspector reviewed two QIRs which indicated that changing of setpoints for the four devices would not be required; however, resolution of this was not finalize Since the inspector was able to verify completion of the majority of these fieldwork open items and the remaining items are being tracked for completion on the "Sequoyah Equipment Qualification Open Items List" and/or the "SQN Startup Activities List (SAL), this inspection follow-up item is close Even though the inspector reviewed the completion of a majority of 1 these items, TVA needs to formally notify the NRC when all Unit 2 EQ ! fieldwork is completed (Unit 1 notification is also required).
