NOTICE OF VIOLATION Tennessee Valley Authority Docket No. 50-328 Sequoyah Nuclear Plant, Unit 2 License No. DPR-79 During an NRC inspection conducted on September 14 through October 16,1998, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedures for NRC Enforcement Actions," NUREG-1600, the violation is listed below:

l l Technical Specification 6.8.1.a requires, in part, that procedures shall be established implemented and maintained covering the activities recommended in Appendix A of i Regulatory Guide 1.33, Revision 2, February 1978," Quality Assurance Program Requirements (Operations)". Appendix A of Regulatory Guide 1.33, Section 9, includes procedures for performing maintenance on safety related components and structures i.e., ice Condenser.

Maintenance instruction (MI),0-Ml-MXX-061-001.0, Revision 6, Ice Condenser Ice Servicing, requires that intermediate deck doors be installed in accordance with l Westinghouse Drawing T189E83 and that bolts used to fasten the intermediate deck door frames to the radial beams be torqued to between 9-11 ft. Ibs. as specified in the subject procedure.

Contrary to the above, as of September 16,1998, requirements of 0-MI- MXX-061-001 had not been implemented in that a significant number of bolts used to fasten the intermediate deck door frames to the radial beams had not been torqued to the specified tolerance as evidenced by loose or missing bolts and by the use of improper washers.

This is a severity Level IV violation (Supplement 1).

The NRC has concluded that information regarding the reason for the violation, the corrective  ;

actions taken and planned to correct the violation and prevent recurrence and the date when l full compliance was achieved is already adequately addressed on the docket in NRC Inspection Report Nos. 50-327/98-13 and 50-328/98-13,- dated Novcmber 24,1998. However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions or your position. In that j case, or if you choose to respond, clearly mark your response as a " Reply to a Notice of l Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region 11 and a copy to the Sequoyah NRC Resident inspector, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).

If you contest this enforcement action, you should also provide a copy of your response to the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, D.C. 20555-0001.
