REPORT NOS. 50-327/87-50 AND 50-328/87-50 Thank you for your submittal of November 4, 1987, regarding our request that you evaluate your response to violations 50-327/84-24-01 and 50-327/84-24-02 and ensure your corrective actions associated with recent operational events encompass the root cause of these 1984 violations.
We have evaluated your submittal and find that we are not in agreement with your response addressing violation 50-327/84-24-01, example e.
The use of caution order tags for personnel safety and plant equipment protection gives us cause for concern; for it is this very use of a caution order tag which contributed to the occurrence of the reactor coolant system spill event of February 1, 1987.
Since this matter is the subject of future enforcement action, it will be addressed accordingly through separate correspondence.
Additionally, with respect to your response to violation 50-327/84-24-01, example f, we find that further explanation of your earlier corrective action is required.
It is our understanding, based on a conversation between Mr. F. McCoy of my staff and Mr. J. LaPoint of your staff, that TVA will provide the necessary explanation in a supplemental response within 30 days of the date of this letter.
