NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-327 and 50-328 Sequoyah Nuclear Plant License Nos. DPR-77 and DPR-79 During an NRC inspection conducted on August 30 through October 10,1998, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedures for NRC Enforcement Actions," NUREG 1600, the violation is listed below:

10 CFR 50, Appendix B, Criterion XI, Test Control, requires, in part, that a test program shall be established to assure that all testing required to demonstrate that structures, systems, and components will perform satisfactorily in service is identified and performed in accordance with written test procedures which incorporate the i

requirements and acceptance limits contained in applicable design documents.

Contrary to the above, the licensee failed to assure that all testing required to demonstrate that safety related circuit breakers would perform satisfactorily in service was identified and performed in accordance with written test procedures which incorporated the requirements and acceptance limits contained in applicable documents.

Specifically:

On May 19,1998, Shutdown Board 1 A1-A Alternate Feeder Breaker 1-BCTB-201-DJ/5B-A failed to perform satisfactorily in service. The failure resulted in a reactor trip and was a consequence of the licensee's failure to assure proper alignment of the breaker's main contacts following maintenance.

On September 11,1998, Shutdown Board 1 A2-A Alternate Feeder Breaker 1-BCTB-201-DM/5B-B failed to perform satisfactorily in service. The failure resulted in the breaker's charging spring motor not actuating as required upon closure of the breaker and inadequate main contact compression and was a consequence of the licensee's failure to assure proper alignment of the breaker's main contacts following maintenance.

This is a Severity Level IV Violation (Supplement 1).

The NRC has concluded that information regarding the reason for the violation, the corrective actions taken and planned to correct the violation and prevent recurrence, and the date when full compliance will be achieved is already adequately addressed on the docket in Inspection Report Nos. 50-327/98-09 and 50-328/98-09. However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions or your position in that case, or if you choose to respond, clearly mark your response as a " Reply to a Notice of Violation," and sent it to the U.S.

Nuclear Regulatory Cc nmission, ATTN: Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator, Region 11, and a copy to the NRC Resident Inspector at the Sequoyah facility within 30 days of the date of the letter transmitting this Notice of Violation.
