Subject: Inspection No. 50-293/85-21

l

l This refers to the special safety inspection conducted by Dr. M. McBride of this

l office on July 16, 1985 through July 30, 1985, at the Pilgrim Nuclear Power Station,

i Plymouth, Massachusetts regarding your response to abnormal surveillance test re-

l sults and to discussions of our findings held with Mr. C. Mathis at the conclusion

of the inspection.

Dased on the results of this inspection, we are concerned about repetitive examples

l wherein plant equipment failed survelliance testing without proper notification

l of operations personnel as required by procedures. As a result, the affected

equipment was not properly declared inoperable and initiation of prescribed cor-

l rective actions was delayed. Consequently, there was an apparent violation of a

l Limiting Condition for Operation on one occasion and surveillance testing required

l by the technical specifications was not promptly initiated on a second occasion.

It appears that a programmatic breakdown in your administrative controls has oc-

curred.

An enforcement conference has been scheduled for August 27, 1985 at the Region I

office to further discuss these problems. At this conference you should be pre-

i pared to discuss your critique of these events and the corrective actions taken.

The need for and nature of appropriate enforcement action relative to the issues

discussed in the enclosed report will be considered after this conference and will

be the subject of separate correspondence at a later time.
