Subject: Inspection Report No. 50-293/87-03

This refers to the routine safety inspection (50-293/87-03) conducted by

Dr. M. McBride, Mr. J. Lyash, and Mr. T. Kim of this office on January 1, to

February 20, 1987 at the Pilgrim Nuclear Power Station, Plymouth, Massachusetts.

Areas examined during this inspection are described in the NRC Region I

Inspection Report which is enclosed with this letter. We identified three

violations of NRC requirements as discussed in the enclosed Notice of Viola-

tion. Normally, we would not cite the incident in the first violation, since

your staff identified the problem. However, I have chosen to issue a Notice of

Violation for this issue because effective corrective actions were not taken

and high radiation access control problems recurred. Please respond to the

Notice within thirty days from receipt of this letter.

Violations identified in the area of high radiation area key control and fire

protection system equipment status appear to indicate that implementation of

improvements in these areas may not have been fully effective. We will con-

tinue to assess the progress of corrective actions. Your continued attention

in these areas appears warrented.

The response directed by this letter and the accompanying Notice is not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reducti.on Act of 1980, PL 96-511.
