Subject: Inspection Report No. 50-293/86-24

This refers to tne special safety inspection conducted by Mr. G. Napuda of this

office on August 4-8, 1986 at Pilgrim Power Station, Plymouth, Massachusetts

and your Braintree, Massachusetts Corporate Offices of activities authorized by

NRC License DRP-35 and to the diccussions of our findings held by Mr. Napuda

with you and other members of your staff at the conclusion of the inspection.

Areas examined during this inspection are described in the NRC Region I

Inspection Report which is enclosed with this letter. Within these areas, the

inspection consisted of selective examinations of procedures and representative

records, interviews with personnel, and observations by the inspector.

Within the scope of this inspection, no violations were observed.

The reactor coolant recirculation pump trip equipment must be designed to perform

its function in a reliable manner as required by 10 CFR 50.62. During this inspec-

tion, the inspectors reviewed the failure history of the recirculation pump motor

i

generator (MG) field breakers which was documented in failure and malfunction

report numbers85-003, 85-013,85-017, 85-157 and 85-165. Similar problems have

been discussed in NRC inspection reports 50-293/83-09, 85-03, 85-14 and 86-21.

The repetitive failures of these breakers call into question the reliability of

.

these devices and if they meet 10 CFR 50.62. Further, the repetitive failures are

! indicative of ineffective corrective actions and an inadequate trending program.

It is our understanding that the most recent MG breaker failure (F&MR 86-157) is

undergoing fr.ilure analysis by your staff. Please provide the results of the

failure and your proposed corrective action to this office .when the results

become available.
