SUBJECT: NOTICE OF VIOLATION (NRC INSPECTION REPORT NOS. 50-327,328/87-24AND50-327,328/87-30)

This refers to the subject Nuclear Regulatory Comission (NRC) inspections conducted at the Sequoyah facility on April 6 - May 5, 1987, and May 6 -
            /
June 12, 1987. The subject reports documenting these inspections and the associated Enforcement Conference were sent to you on June 4, 1987, and July 20, 1987, respectively. The inspections included a review of the circumstances surrounding the February 1, 1987 and the April 29, 1987 reactor coolant system spill events, and observations of ongoing testing evolution As a result of these inspections, five failures to comply with NRC regulatory requirements were identified and, accordingly, NRC concerns regarding the inspection findings were discussed by J. A. Axelrad, Deputy Director, Office of Special Projects and G. G. Zech, Assistant Director, Inspection Programs, TVA Projects, Office of Special Projects - NRC, with C. C. Mason, Deputy Manager of Nuclear Power-TVA, in an Enforcement Conference held on June 18, 198 The violation described in the enclosed Notice of Violation involves several examples of inadequate procedure implementation and identifies occurrences which demonstrate a lack of control over testing evolutions, and system and equiprnent status. We believe that issuance of this Notice of Violation is necessary to obtain the management attention necessary to resolve the underlying problem. Your corrective action will be specifically reviewed during the heat up phase and NRC operational readiness evaluatio You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response. In your response, you should document the specific actions taken and any additional actions you plan to p. event recurrence. After reviewing your response to this Notice, including your proposed corrective actions, the NRC will determine whether further NRC enforcement action is necessary to ensure compliance with NRC regulatory requirement \
