This' refers to-the special inspection conducted by the NRC Augumented Inspec-

. tion Team (AIT) at your Sequoyah facility during the period October 11-19,

'

1990.

The inspection included a review of events- that led to the discovery of

multiple failures of ' Main' Steam Check Valves .on October 8,1990.

At the

. conclusion of the, inspection,-the. findings were discussed with those members.of'

'your staff identified in the enclosed' inspection report.

The AIT concluded that the multiple failures were the result of inadequate

h

> recognition ' and correction ,of deficiencies in the original design and

l

inadequate analysis of.a recent design modification.. The recent design change

jl

l

was: concluded to' be the principal contributor to the rapid progression of valve -

failures;

Excessive reliance, appeared to be placed -on vendor concurrence

l

rather than detailed design evaluation of the change.

LThe enclosed' copy of the AIT~ report identifies the areas examined during this

inspection. - . Within these areas, the inspection ' consisted -.of selective

examinations of . procedures . and representative records, interviews with
