SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/89-19 AND 50-328/89-19)

This refers to the Nuclear Regulatory Commission (NRC) routine monthly

' inspection conducted by K. Jenison from July 6, 1989 thru August 5, 1989. At

the conclusion of the inspection, the findings were discussed with those

members of your. staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. These

areas included operational safety verification, maintenance activities,

surveillance activities, and followup of plant events. Within these areas,

the' inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of

activities in progress. As a result of these inspections, a weakness in the

accuracy of information provided to the NRC emergency response center was

identified as described in paragraph 9.b of this report. This weakness, which

is similar to a violation issued in NRC Inspection Report 327, 328/88-34, was

discussed with the Sequoyah Site Director. Corrective action will be reviewed

during future NRC inspections.

The inspection findings indicate that certain activities appear to violate NRC

requirements. The identified violations involved a failure to comply with a

Technical Specification Action Statement and a failure to implement an abnormal

operating instruction. The violations, references to pertinent requirements,

and elements to be included in your response are described in the enclosed

Notice of Violation.

Your attention is also invited to the unresolved items identified in the

Inspection Report. Two of the unresolved items address apparent weaknesses in

your control of the system configuration of safety related systems, and the

availability of adequate procedures to implement TS action statements.

Additional NRC review of these issues will be conducted during future

inspections.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2

Title 10, Code of Federal Regulations, a copy of this letter, its enclosures,

and any reply will be placed in the NRC's Public Document Room. }
