(NRC INSPECTION REPORT N05, 50-327/90-22 AND 50-328/90-22)

This refers to the NuMear Regulatory Commission (NRC) routine monthly

-_ inspection conducted by P Harmon from June 6,1990 thru July 5,1990.

The

' . inspection included a review of activities authorized- for your Sequoyah

facil!ty.

At the conclusion of the inspection, the findings were discussed

'

with those members of your staff identified in the enclosed inspection report..

Areas examined during the inspection are identified in the report.

Within

_these areas, the inspection consisted of selective examinations of procedures

' _

iandTrepresentative records, interviews with personnel, and observation of

,iactivities'in_ progress.

The_inspe'ction findings-indicate that certain activities appear to viciate-NRC

requirements.

The violations, references to pertinent requirements, and

telemee,ts to be included in your response are described in-the enclosed Notice.

cThe violation described in the enclosed Notice is similar to a violation

contained:in the Notice sent to you by our letter dated March 14, 1988.

. Recurring violations are of particular concern because the NRC expec_ts

,J11censees to learn frr, their past : failures and to take effective corrective

actions.

Although NRC does not normally consider monetary civil penalties for

iSeverity. Level IV violai-ions, the Enforcement Policy states ' hat 'such

penalties may be imposed for Severity Level IV violations-that 6,e similar to

previous . violations _for which the licensee did not take effective corrective

Taction.

.In' this case, we have decided not to hold an enforcement conference

nor;to propose-a civil penalty because significant changes in plant management

? occurred : since the--previous violation and the time frame between the

occurrences -of the violation was very close to the two year criteria. However,

.ofl additional concern is why your QA department had not identified this' problem

.
