SUBJECT: NOTICE OF VIOLATION, NRC SPECIAL INSPECTION REPORT ON SEQUOYAH

SHUTDOWN BOARD OVERV0LTAGE (NRC INSPECTION REPORT NOS. 50-327/89-14

A*fD50-328/89-14)

This refers to the Nuclear Regulatory Commission (NRC) team inspection conducted

at the Sequoyah Nuclear Plant on April 24 through April 28, 1989. The inspection

was announced by letter dated March 7, 1989. The inspection was conducted to

determinn the adequacy of procedures for managing overvoltage conditions and

also to ettermine whether current plant procedures were adequately implemented

to protect sensitive electrical equipment from damage that would result from

overvoltage. An overvoltage condition existed at the Sequoyah site during diesel

testing on January 21, 1989 during which the Tennessee Valley Authority (TVA)

took no immediate corrective actian. The NRC staff discovered this overvoltage

condition on January 22, 1989 when TVA reported to the staff the failure of

diesel generator 2A-A to limit tre.:1ent overvoltage on the shutdown' boards to

within the Technical Specification limits during load rejection testing.

Previous overvoltage conditions at the site from 1980 through 1986 were identi-

fied in TVA's corrective action program included in the Employee Concerns Special

Programs Element Report 30202.

The inspection determined (1) that plant personnel failed to follow established

procedures, and (2) that some procedures provided inadequate instructions to

plant personnel for assuring proper voltage en the shutdown boards. Furthennore,

the analysis perfonned in conjunction with Element Report 30202 to assess the

effects of overvoltage on the electrical equipment was inadequate, and the root

cause of overvoltage events was not properly recognized in order to ensure adequate

corrective action. The staff's concerns about the Element Report are considered

inspector followup items.

The failure to follow established procedures is a violation of NRC requirements.

Please include in your response to Violation 50-327/89-14-01, 50-328/89-14-01 a

discussion on how the inadequate procedures identified in Section 6.1 of the

inspection report contributed to the violation and what actions you have taken

to correct this situation. The violation, reference to pertinent requirements,

and elements to be included in your response are described in the enclosed Notice

of Violation.
