Dear Mr. Scalice:

. This refers to the inspection conducted on November 22,1998 - January 2,1999, at the
Sequoyah facility. The enclosed report presents the results of this inspection.
'
During the inspection period, your conduct of activities at the Sequoyah facility was generally
i
- characterized by satisfactory operations. Improvements were identified in the maintenance and
i
.
refurbishment of electrical breakers.' Engineering evaluations related to ice condenser
modifications were properly evaluated for conformance with the Updated Final Safety Analysis .

Report.

Based on the results of this inspection, the NRC has determined that violations of NRC
- requirements occurred. The violations are cited in the enclosed Notice of Violation (Notice) and
the circumstances surrounding them are described in the subject inspection report. The
violations involved (1) two examples of failure to comply with emergency operating procedures
- (EOPs) following a reactor trip and (2) failure to enter Technical Specification (TS) 3.0.3 when
,
- the limiting condition for a reactor coolant system flow instrumentation TS requirement was not
met. These violations are of concem because they indicate a weakness in licensed operator's
understanding and adherence to TSs and EOPs.
You are required to. respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necassary to ensure compliance with
regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room.
