M. W. Alsworth, Manager, Reactor Safety Enforcement Conference On July 9,1986, an enforcement conference was held at the PGE corporate office in Portland, Oregon, with the individuals listed in paragraph 1 above. The conference was in regard to a special inspection of quality control activities conducted on May 13 - June 12, 1986. The inspection is described in inspection report 50-344/86-24. A copy of the inspection report was sent to the licensee on July 1, 198 l Mr. Kirsch informed the licensee that the purpose of the conference was to discuss the apparent violation of rebulatory requirements identified during the recent inspection of quality control inspection activities and its underlying causes. Mr. Kellund discussed the details of the inspection findings as described in the" inspection repor Mr. Olmstead acknowledged the facts and. circumstances that allegedly constituted the violation of the regulatory requirements and described completed and proposed corrective actions to ensure compliance with the regulatory requirements. He stated that, in the future, the quality assurance organization would perform early surveillances of contract

  • quality control inspectors for outages to assure they have a satisfactory level of knowledge and an understanding of licensee expectations of the Additionally, he stated that the licensee quality control organization *

.Mr.-Kirsch 1 stated that the maintenance personnel involved in the activity bear some responsibility for the occurrence.- He noted~that the-maintenance workers should not have worked past quality control-

' inspection points without ensuring that a quality control inspection was performed. Mr. Orser agreed with Mr. Kirsch and stated that maintenance personnel have been counseled regarding this issu Mr. Martin said that PGE management needs to ensure that contract

. personnel brought _onsite fully understand their duties and-responsibilities, and that management needs to effectively, communicate its expectations to them if this is to occu >

Mr. Withers agreed with Mr. Martin that. the contract quality control inspectors brought onsite for the' refueling outage;had not'been adequately indoctrinated. He stated that during outages it has been difficult to get these individuals onsite'and properly indoctrinated due to time constraints. He indicated that._for future outages, PGE plans to

' bring a contractor representative onsite a number of weeks before the outage to allow him to become familiar with the plant and with the licensee organization, and to allow PGE to become familiar with him and
