Enforcement Conference On March 27, 1986, an enforcement conference was held at the NRC Region V office in Walnut Creek, California, with the individuals listed in paragraph 1, above. The conference was in regard to a special inspection of the Trojan control room emergency ventilation system. This inspection covered the period January 28-30 and February 24-28, 1986, and is described in inspection report no. 50-344/86-06. The conference was announced in a letter to the licensee dated March 20, 1986.
Mr. Martin informed the licensee that the purpose of the conference was to discuss the apparent violations of regulatory requirements identified during the recent inspections of the control room emergency ventilation system and the underlying cause or causes.
Mr. Martin recapped the highlights of some significant PGE engineering problem areas. These problem areas (also identified in SALP) related to the following Trojan plant equipment: Diesel Generator, Barton Pressurizer Pressure transmitter, Ventilation system for the fuel and auxiliary building, Pressurizer scaling factors, RHR pump performance limits and the Auxiliary Feedwater system reliabilit r

Mr. Pate discussed the issues identified during the inspection and summarized in the inspection report. Then Mr. Pate discussed the significance of the key issues and the resulting inoperability of the control room emergency ventilation system due to design, construction, procedural, and equipment deficiencies.
Mr. Withers responded to Mr. Martin's opening remarks and continued with an explanation of the "B" train leakage from a missing cooler housing corner and the excess makeup flow in the
"B" train as well as the low flow in the "A" train.
Mr. Withers also discussed the drain line that the "A" and "B" trains shared.
Mr. Keiter discussed the "as found" configuration with a flow diagram of the system and flow measurements that were taken. The licensee stated that the inconsistencies between the FSAR and TMI item III.D.3.4 and the surveillance and operating procedures were caused in 1975 when the system was changed to accommodate the " Murphy-Kemp Analysis".
Mr. Lentsch stated that a PGE analysis of the common drain line and the leak in the
"B" train showed that a dose of 194 rem to the Thyroid could be received by a control room operator unless the operator took such action as taking Potassium Iodide pills or donning respirators.
Mr. Withers agreed with Mr. Martin that the control room emergency ventilation system, in the "as found" configuration, would not have performed its intended function and further stated that operator action would have been necessary to satisfy GDC-19.
Mr. Withers further stated that the NRC and PGE identified deficiencies have been corrected.
Mr. Olmstead indicated that the past Quality Assurance program audits should have identified some of the system deficiencies through resolution of the inconsistencies in the FSAR and Murphy-Kemp analysis but failed to do so because they were not comprehensive enough to identify the inconsistencies.
Mr. Olmstead said "the current QA program does business much better" and is performing a total system review (assessment) on a priority schedule as resources allow. He further indicated that this review will take several years to complete and once completed will restore the appropriate confidence in the plant systems.
Mr. Martin said he expected PGE Engineering to understand how their systema work and to ensure that the system design criteria does not get lost over the years. PGE should ensure that base line data that gets changed through modification are updated to reflect these modifications.
The licensee stated that the engineering staff has been increased both at corporate and site and that the responsible system engineer program that has been underway is yet to be completed.
Mr. Keiter said that PGE is currently trying to " catch up" to where PGE expects and wants to be with regard to the level of systems expertise and review.
Mr. Martin said that the " base line" that PGE is currently generating through the QA assessment and the development of and review of systems by responsible system engineers appears to be "a must" and encouraged PGE to complete this review and assessment on a timely basis.
