2. Exit Interview

The inspection scope and findings were summarized on February 27, 1987,

with those persons indicated in paragraph 1 above. The inspector

described the areas inspected and discussed in detail the inspection

findings. No dissenting comments were received from the licensee. The

following new item was identified during this inspection.

Conducted During 1986 paragraph 5.e(2).

The licensee did not identify as proprietary any of the materials provided

to or reviewed by the inspector during this inspection.

a. (Closed) Violation 348/83-21-01 and 364/83-19-01, Inadequate

Surveillance Test Procedures for Fire Detection System, Fire

Protection Control Valves and CO 2 Hose Reels. Procedures

FNP-1-STP-131.02, FNP-0-STP-56.0 and FNP-0-STP-129 have been revised

to include the required surveillances. Therefore, this item is

closed.

b. (Closed) Violation 364/83-19-03, Fire Stops are Not Provided for

Vertical Cable Trays. The licensee performed a detailed evaluation

of all vertical cable raceways (Production Change Notice B-83-1470

for Unit 1 and B-83-2454 for Unit 2). This evaluation identified a

number of raceways in which fire stops were required but were not

provided. All of the required fire stops were installed prior to

May 7, 1984. The inspector reviewed the following drawings to verify

that the required fire stops and sealing requirements had been

installed: Unit 1, D-350899 and D-350913; and Unit 2, D-356114 and

D356134. Therefore, this item is closed.

These procedures comply with the NRC supplemental guidelines of the

document entitled " Nuclear Plant Fire Protection Functional Responsi-

bilities, Administrative Controls and Quality Assurance" dated

June 14,1977. However, several items in the administrative pro-

cedures do not meet the current NRC guidelines -and requirements of

10 CFR 50,' Appendix R. These include the failure to limit open flame

,

permits to 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> while plant 's in operation, no yearly physical

examinations for fire brigade members and lack of detail fire

fighting strategies. These issues have previously been reviewed and

accepted by NRR, and, therefore, are not applicable to Farley.

The above surveillance procedures were reviewed to determine if the

various test outlines and inspection instructions adequately imple-

, ment the surveillance requirements of the plant's fire protection

'

Technical Specifications. In addition, these procedures were

reviewed to determine if the inspection and test instructions

followed general industry fire protection practices, NRC fire

protection program guidelines and the guidelines of the National Fire

Protection Association (NFPA) Fire Codes. Based on this review, it

appears that the above procedures are satisfactory.

The surveillance test record data and testing frequency associated

with the above fire protection system surveillance test / inspections

were found to be satisfactory with regard to meeting the requirements

of the plant's Fire Protection Technical Specifications.

d. Fire Protection Audit

The most recent audit reports of the Farley Fire Protection Program

were reviewed. These audits were:

Annual Fire Protection / Loss Prevention Audit of August 9, 1985.

-

SAER QA Fire Protection Audit of August 11, 1986.

-

Triannual (PLC) Fire Protection Audit of October 7, 1986.

The audits identified several fire protection program improvements.

The licensee has either implemented the corrective actions associated

with these audit findings or were evaluating the items to determine

the appropriate corrective actions. The licensee appears to be

taking the appropriate corrective actions on the audit findings.

These audits appeared to meet the intent of the Technical Speci-

fications; however, the 'QA audit program, procedure SAER-WP-12, Fire

Protection Audit Program, did not reference NRC Generic Letter 82-21,

Technical Specifications for Fire Protection Audits, to assure that

l all of the required elements were included in the QA audit checklist

i and future audit. The licensee agreed to incorporate this generic

letter in the reference sections of procedure SAER-WP-12. This will

be verified during future NRC inspections.

! e. Fire Brigade

(1) Organization

The total station fire brigade is composed of approximately 144

personnel from the operations, security and slant chemistry

staff. The un duty shift fire brigade leader " s normally one of

.

the shift foremen and the remaining four fire brigade members

l are composed of two plant operators, one security officer and

one person from the chemistry group. An auxiliary brigade

composed of four employees from the maintenance, warehouse and

security staff is also available. It appears that sufficient

manpower is available to meet both the operational and the fire

l brigade requirements of. the plant's Technical Specifications.

(2) Training

The inspector reviewed the training and drill records of 13

brigade members for 1986. The recor_ds reviewed indicated that

each of these leaders and members had attended the required

training and participated in the required number of drills. The

fire brigade training records which were inspected were found

satisfactory.

The inspector reviewed the shift f' ire brigade drills conducted

during 1986 and noted that 18 drills were conducted. Three

drills were conducted the first quarter, none the second

quarter, one the third quarter and 14 the forth quarter. Farley

Nuclear Plant Training Plan, FNP-0-AP-45 Appendix Q states that

the fire brigade training program will consists of formal

lectures and six brigade drills per quarter. The failure to

conduct the required number of drills is identified as Violation

348, 364/87-07-01, Inadequate Number of Fire Brigade Drills

Conducted During 1986.

f. Plant Tour and Inspection of Fire Protection Equipment

(1) Inspection of Fire Brigade Manual Fire Fighting Equipment

The inspector performed an inspection of the fire brigade

equipment, consisting of fire hose, nozzles, tools, and miscel-

laneous equipment. This equipment is stored at the fire brigade

equipment response location located in Unit 1 auxiliary building

(155' elevation), Unit 1 turbine building (155' elevation),

service water structure, river water structure and diesel

generator building. A total of 14 sets of turnout gear (coats,

boots, helmets, etc.) and miscellaneous equipment are stored at

the fire brigade equipment responses location.

The designated fire brigade equipment appeared to be properly

maintained and stored in a ready condition.

(2) Outside Fire Protection Walkdown

The inspector verified that the two water storage tanks con-

tained sufficient water to meet the requirements of the

Technical Specifications. The three fire pumps were inspected

and found to be in service. The diesel fuel tanks for the

diesel driven fire pumps were approximately 75% full of fuel

which met the requirements of the Technical Specifications.

However, the electric driven fire pump was in continuous

operation to maintain system pressure due to a number of leaks

in the underground water distribution piping system. These

leaks exceeded the capacity of the normal jockey pump. The

alarm panel in the control room, which indicates all fire anc'

trouble alarms from the fire pump house, had been placed in the

off position in order to maintain the fire protection system

annunciator light on the main control room board in operation

for the other fire protection and detection systems. This

resulted in the lack of a trouble or fire alarm initiation in

the event any alarm was initiated from the fire pump house. The

loss alarms included: pump house fire alarm, fire pump running,

fire pump one start failure, fire pump two start failure, fire

pump three start failures and fire protection system low water

pressure. The licensee stated that other alarms are available

to compensate for these loss alarms and that the alarm panel was

to be restored to normal as soon as the leaks were repaired. If

'

the leaks were not repaired soon, the licensee indicated that

consideration would be given towards implementing the " abnormal

or jumper installation control procedures" in order to restore

the alarm panel to service. This item will be reviewed during

future NRC inspections.

protection water supply system were inspected and verified to be

properly aligned and locked in position:

The equipment houses contained the minimum equipment requirement

of that specified by NFPA-24, Private Fire Service Mains and

Their Apparentness, and/or the FSAR commitments. The equipment

appeared to be adequately maintained.

A tour of the exterior of the plant indicated that sufficient

clearance was provided between permanent safety related

buildings and structures and temporary buildings, trailers, and

other transient combustible materials. The general housekeeping

of the areas adjacent to the permanent plant structures was

satisfactory.

(3) Permanent Plant Fire Protection Features

A plant tour was made by the inspector. During the plant tour,

the following safe shutdown related plant areas and their

related fire protection features were inspected: diesel

generator building and non radiation areas of the auxiliary

building.

The fire / smoke detection systems, manual fire fighting equipment

(i.e., portable extinguishers, hose stations, etc.), fixed

suppression systems, and the fire area boundary walls, floors

and ceiling associated for the above plant areas were inspected

and verified to be in service or functional. Based on this

inapection, it appears that the fire protection features

associated with the above plant areas are satisfactorily main-

tained.

The plant tour also verified the licensee's implementation of

the fire prevention administrative procedures. The control of

combustibles and flammable materials, liquids and gases, and the g

general housekeeping. were found to be very good in the areas

inspected. A welding operations was observed in the diesel

generator building. An approved " Hot Work" permit had been

issued for the welding operation and the work practices met the

licensee's fire prevention control procedures.

(4) Appendix R Fire Protection Features

The inspector visually inspected the fire rated raceway fire

barriers required for compliance with Appendix R, Section

III.G.2 in the following plant areas:

Based on the inspector observations of the above raceway fire

barrier enclosures, it appears that the one hour fire barrier

integrity associated with the above fire barrier assemblies was

being properly maintained in a satisfactory condition.

Twelve eight-hour emergency lighting units installed between the

control room and the Units 1 and 2 remote shutdown panel rooms

were inspected. These units were in service, lamps properly

aligned and appeared to be properly maintained.

Except as noted above, within the areas inspected, no additional viola-

tions or deviations were identified.

6. Inspector Followup Items (IFI)

a. (Closed) IFI 348/83-21-02, Missing Surveillance Datii Sheet for

October 1982 of Procedure FNP-0-STP-126.02. This completed procedure

could not be located. However, the control roon records indicated

that this surveillance was completed but was apparently never

received by the document control center. This item is closed.

b. (Closed) IFI 364/83-19-02, Fire Hose Stations for Unit 2 Cable

Tunnels are not Included in the Technical Specifications. These hose

stations are now part of the Technical . Specifications. This item is

closed.
