The NRC has received an allegation regarding operating procedures at the Braidwood Station. The allegation consisting of three issues c.in be summarized as follows:

1. The procedure development and review system lacked adequate checks and balances and were not properly implemented. Two specific areas of concern noted were:

a) Incorporation of review comments were at the discretion of the author since there was no provision for the reviewer to ensure resolution of his comments;

b) That the plant review committee approved procedures which were not in final typed form with no provision for subsequent administrative review to assure accuracy.

2. Procedure quality was poor as evidenced by the following comments:

a. Procedures lacked adequate precautions for equipment operation.

b. Procedures did not standardize the method for placing control valves in service.

c. Procedure for providing make-up water to the Primary Water Storage Tank (PWST) was inadequate for the following reasons:

• Cordell Reed January 16, 1986 (2) Procedure was incomplete because it did not contain instructions for shJtting down the preheater after filling the PWST.

3. Because the PWST procedure had been in use for three years with these deficiencies this raises questions as to whether or not plant operators have been following it. If the procedure has been followed then the PWST make-up water preheater has been repeatedly thermally shocked and its service life significantly shortened.

The NRC expects that Commonwealth Edison Company will evaluate the concerns, perform the necessary audits to determine their validity and significance, and take appropriate action as necessary to address the specific concerns and root Causes.

We request that the results of your review and disposition of these concerns be submitted to Region III upon completion and that documentation supporting your results be maintained onsite for further inspection by Region III during our routine inspection program.
