in which emergency
core cooling system (ECCS) operability was affected by a mispositioned-
isolation valve. Details of the event, as well-as details of two previous
events involving mispositioned ECCS valves, were discussed during an
enforcement conference held in the NRC Region III office in Glen Ellyn,
,

Illinois on July 11, 1989. The enclosed copy of our report identifies areas
discussed during the meeting.

As discursed during the enforcement conference, although the events involved
'
oifferent circumstances, we are concerned with the effectiveness of the
controls you presently have in place to ensure the continuous operability
of all ECCS equipment. We understand you are presently reviewing these
controls and request that you discuss any changes you have made or plan to
l
make to your program in your response to this violation.

Based on the information obtained during the inspection and provided at the
i
enforcement conference, we have concluded that the safety significance of the
most recent valve mispositioning event, even when considered in conjunction-
with the two previous events, does not warrant escalated enforcement' action.
However, Braidwood may be subject to escalated enforcement upon discovery
of additional valve mispositioning which affects ECCS operability.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,
. Title 10, Code of Federal Regulations, a copy of this letter and-its
enclosures will be placed in the NRC Public Document Room.
