Dear Mr. Kingsley:

On July 21,1999, the NRC completed an inspection at the Braidwood, Byron, and Quad Cities

reactor facilities, in our March 26,1999, letter forwarding the results of our Plant Performance

Review, we identified that recurring configuration control issues represented a challenge at all

three stations. The purpose of this regional initiative inspection was to evaluate your

implementation of configuration control processes and to evaluate the effectiveness of your

corrective action approach to the continuing configuration control occurrences at each of these

stations. Within these areLs, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observations of activities in

progress. The enclosed report presents the results of that inspection.

While we recognize that your performance in controlling the configuration of plant systems and

equipment has improved, it is important to emphasize that this area remains a challenge to your

staff. We noted that continuing configuration control occurrences were primarily attributable to

human performance deficiencies with the largest contributor involving maintenance activities.

We determined that action plans had been developed at all three sites to address configuration

control weaknesses. However, the plans were not totally effective for several reasons

including, not completing the action plan, and not evaluating the effectiveness of the

implemented portion of the action plan. We also determined that station personnel, including

first line supervisors, generally did not recognize and understand the configuration control and

human performance issues that existed at their site, in addition, we noted that some first line

supervisors were not knowledgeable of all aspects of the configuration control processes and

were not in the field sufficiently to reinforce station standards and expectations.
