Gentlemen:

This refers:to the-routine safety inspection' conducted by Dr. R._B. Holtzman

and Mr. M. A. Kunowski of.this office from July 3 through September 1,1989,

of activities at Braidwood Station,_ Units 1 and 2 authorized by NRC Operating.

Licenses No. NPF-72 and NPF-77, and to the discussion of our findings with

Mr. R. Querio and others at the conclusion of the onsite inspection. It

also refers to subsequent' telephone discussions with Messrs. D. Ambler and

K. Weaver on August 11,'1989, with Mr. D. O'Brien and others on August 15, ,

1989, and with Dr. J. ' Golden on August 30, 1989.

The enclosed copy.of- our inspection report identifies' areas examined during

the inspection. Within these areas, the inspection' consisted of a selective

examination of. procedures and representative records, observations, and

interviews.with personnel.

During this inspection, certain of your activities appeared to be in violation

of_NRC' requirements, as specified in the enclosed Notice. A written ~. response

is required. We are particularly concerned about Violation No. 2, in which

-

- severalL layers of oversight failed in their intended purpose to identify a

recorded survey which exceeded a clearly stated limit. We are also concerned

that your personnel may not have understood the purpose of the vehicle survey-

requirement and the 10 mR/ hour at 2 meters limit, and the training weakness

that the lack of understanding implies.
