This refers' to the routine safety inspection conducted by Mr. R.. A. Hasse,

Ms. A. M. Bongiovanni, and Ms. V. P. Lougheed of. this office on February 27

through March 16, 1989, of activities at Byron Nuclear Power Station, Units li

'

and.2 authorized by NRC Operating Licenses No. NPF-37 and NPF-66 and to the

discussion of our findings with Mr. R. Pleniewicz and others of your staff

at t~ne conclusion of the inspection.

The purpose of this inspection was to determine if design changes and

modifications and the dedication of commercial grade equipment for ,

safety-related applications were being conducted in accordance with

'

regulatory ~ requirements. Within these areas, the inspection consisted  !

of a selective examination of procedures and representative records,

observations, and interviews with personnel.-

The results of this inspection indicated some weaknesses in the areas of design

control and post modification testing. It appeared that this was due, in part, ,

'to a weakness in communications between the large number of organizations  ;

involved in the design change process. Several examples of problems noted i

were: failure to seismically qualify several valves installed in a j

modification to MSIV accumulator test connections; failure to convey critical i

preventative maintenance requirements for diesel general governor circuit

relays; and, failure to fully test a modification installing a time delay

relay.in the auxiliary feedwater pump trip logic.

We were also concerned with the apparent lack of understanding of the intent

of 10 CFR Part 50, Section 50.59 and the lack of a well-structured program ,

,

for dedicating commercial grade equipment for safety-related applications.

We were encouraged by our review of several recent audits performed by your

quality assurance organizations in that they had identified several concerns

similar to'those identified during this inspection. This indicates that your

audit program is effective in identifying problems.

During this inspection, certain of your activities appeared to be in violation

of NRC requirements, as specified in the enclosed notice. A written response

is required.

We also request that you respond to the open items identified in Paragraphs

3.b(1) (design control), 3.b.(2) (Post Modification Testing), and 4.c

(dedication of commercial grade equipment for safety-related applications) of

the attached Inspection Report. In responding, we request you address how

such problems are occurring in view of your Modification Program enhancements.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget

as required by the Paperwork Reduction Act of 1980, PL 96-511.

We will gladly discuss any questions you have concerning this inspection.
