SUBJECT: BYRON INSPECTION REPORT 50-454/99010(DRP); 50-455/99010(DRP)

Dear Mr. Kingsley:

On August 2,1999, the NRC completed an inspection at the Byron 1 and 2 reactor facilities. i

The enclosed report presents the results of that inspection.

l

Dunng this inspection period, control room operations were conducted in a safe, professional, i

and controlled manner. We observed that operators generally adhered to the station's l

standards for control room conduct, procedural adherence, annunciator response, and use of l

three-way communications. We also noted that observed maintenance activities were

coniucted well. Maintenance personnel were knowledgeable of the tasks and professionally i

completed tne work. In particular, maintenance associated with the replacement of a degraded

cell in the Unit 2 train B safety-related 125 volt battery was planned and executed well.

We identified that appropriate post maintenance testing requirements were not incorporated

into maintenance work instructions for the Unit 2 diesel driven auxiliary feedwater pump. The

testing performed fol!owing replacement of control power diodes in the pump's control circuit

was not adequate to demonstrate that the control circuit would have performed as designed.

We, also, noted that your corrective action process was not effective to preclude similar

inadequate testing of an essential service water make-up pump 4 weeks later.

We have also observed that station personnel have missed prior opportunities to identify

multiple rnaterial condition deficiencies in the plant and have failed to enter those material

condition deficiencies into the your corrective maintenance program for repairs. Although the

safety consequences of these deficiencies were minor, if left uncorrected, these problems could

potentially impact the ability of the systems to perform their required function.

Based on the results of this inspection, the NRC has determined that one violation of NRC

requirements occurred. This violation is being treated as a Non-Cited Violation (NCV),

consistent with Appendix C of the Enforcement Policy. The NCV is described in the subject

inspection report. If you contest the violation or severity level of the NCV, you should provide a

response within 30 days of the date of this inspection report, with the basis for your denial, to

the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington DC '

20555-0001, with copies to the Regional Administrator, Region Ill, and the Director, Office of

Enforcement, United States Nuclear Re'gulatory Commission, Washington, DC 20555-0001.

In accordance with 10 CFR 2.790 of the NRC's Rules of Practice," a copy of this letter, its

enclosure, and your response, if you choose to provide one, will be placed in the NRC Public

Document Room.
