SUBJECT: BYRON INSPECTION REPORT 50-454/99004(DRP); 50-455/99004(DRP)

Dear Mr. Kingsley:

On May 10,1999, the NRC completed an inspection at the Byron 1 and 2 reactor facilities. The

enclosed report presents the results of that inspection.

During this inspection period, control room operations were consistently performed in a safe

and professional manner. This was particularly evident during the Unit 1 startup following

refueling outage B1R09. Supervisors demonstrated good command and control, directed

. reactivity manipu!ations, and provided effective oversight of the startup activities. We observed

l that operators adhered to the station's standards for control room conduct, procedural

I

adherence, annunciator response, and use of three-way communications.

There were two notable examples during this inspection period wherein maintenance personnel

!

failed to follow station procedures for the control of maintenance activities. In the first example,

mechanical maintenance personnel failed to have a final cleanliness inspection quality control

hold point completed prior to re-assembly of the 1 A reactor coolant system loop stop isolation

valve. This resulted in foreign material left in the reactor coolant system following the

j maintenance. In the second example, mechanical maintenance personnel adjusted the packing

L on the 18 main steam isolation valve without approved work instructions and operations

l department authorization for the activity. We recognize that you have implemented corrective

l actions for similar human performance issues over the past year and that the effectiveness of

j those actions have not been fully demonstrated.

l Based on the results of this inspection, the NRC has determined that five violations of NRC

- requirements occurred. These violations are being treated as Non-Cited Violations (NCVs),

consistent with Appendix C of the Enforcement Policy. These NCVs are described in the

subject inspection report. If you contest any of the violations or severity level of these NCVs,

you should provide a response within 30 days of the date of this inspection report, with the

basis for yo r denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk,

Washington DC 20555-0001, with copies to the Regional Administrator, Region Ill, and the

Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington,

DC 20555-0001.
