BYRON INSPECTION REPORT 50-454/99002(DRP); 50-455/99002(DRP)
Dear Mr. Kingsley:

On February 16,1999, the NRC completed an inspection at the Byron i and 2 reactor facilities.
The enclosed report presents the results of that inspection.
i

During this inspection period, routine operations of the facility were conducted in a professional,
safe and controlled manner. Operators responded appropriately to alarms, closely monitored
main control room panels, were knowledgeable of plant conditions, properly used procedures,
and generally used three-way communications. We also noted that maintenance work
instructions were generally commensurate with the skill level of maintenance personnel and
were sufficient to properly perform the maintenance activities, with one notable exception.
Maintenance work instructions for the diesel generators did not contain any guidance on
priming the diese1 generators fo!!owing maintenance on the fuel oil system and the instructions
lacked appropriate guidance for the assembly of some mechanicaljoints.
We continue to be concemed with the trend of configuration control and human perforrnance
,
errors at Byron Station. Numerous configuration control and human performance issues have
been documented in NRC inspection reports over the past year and most of these issues had to
do with nonsafety-related equipment. We recognize you have been aggressively identifying
configuration control and human performance issues and you have initiated corrective actions
'
to address these problems. However, your corrective actions have not been totally effective at
arresting these negative performance trends. Accordingly, we are concerned with the
effectiveness of your corrective action program in these areas.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice l' a copy of this letter and its
enclosure will be placed in the NRC Public Document Room.
