SUBJECT: NRC INSPECTION REPORT REPORTS 50-454/98024(DRS);

50-455/98024(DRS) -

Dear Mr. Kingsley: ,

On December 4,1998, the NRC completed an inspection at your Byron Nuclear Generating

Station. The enclosed report presents the results of that inspection. >

The purpose of the inspection was to determine if licensed operator requalification training

activities were conducted in accordance with NRC requirements. The inspectors concluded the

licensed operator requalification program was generally implemented in accordance with

10 CFR 55 requirements with some weaknesses.

, Prior to the scheduled NRC inspection, Comed personnel from the Braidwood Production

Training Center conducted an in-house inspection of the requalification program at Byron i

Station. NRC inspectors reviewed the results of the inspection and noted the considerable

effort put forth by the inspection team as evidenced by the high quality report and number of ,

findings generated from the inspection. Many of the issues identified in the in-house report are j

also identified within this report to ensure proper NRC tracking of those issues. Of particular l

concern was the ' poor quality of the Byron Station training program sample plan. Title 10 CFR

55, allowed you to implement a Systematic Approach to Training (SAT) program in lieu of the

required NRC training program outlined in 10 CFR 55, provided that you properly maintain t .e i

program. The inspectors determined that your program had implemented the basic elements of

the SAT program, but the controlling element (sample plan) of the program was poorly

implemented.

Despite the weakness of the sample plan, the training department's instructors have j

implemented the requalification training program and have been generally successful in

providing operators with the skills necessary to safely operate the facility. During the inspection

- week, however, one crew failed the annual operating test based on significant command and

control weaknesses. This was in contrast with the other crews evatuated during the week

where only minor deficiencies were observed and one crew exhibited strong command and

control capabilities.

During control room observations, the inspectors determined panel walkdowns and shift

turnovers were conducted appropriately.
