BYRON INSPECTION REPORT 50-454/98020(DRP); 50-455/98020(DRP)

Dear Mr. Kingsley:
On November 23,1998, the NRC completed an inspection at the Byron 1 and 2 reactor
facilities. The enclosed report presents the results of that inspection.
During this inspection period, control room operations were performed in a professional
manner. Operators were attentive, knowledgeable of plant conditions, and responded
appropriately to alarms and plant transients. We also noted that maintenance work instructions
were generally commensurate with the skill level of maintenance personnel, with two notable
exceptions. The first example resulted in the inadvertent rupture of a temporary inflatable pipe
plug which was an isolation boundary for maintenance on the essential service water system;
and the second example resulted in the 1 A emergency diesel generator being rendered
inoperable due to fouled lubricating oil stainers.
We are concerned with the five human performance errors that occurred during this inspection
period. Each of these errors represented multiple individual failures, most of which were due to
either inadequate independent verifications or ineffective self checking techniques. These
errors also indicated that the underlying problems were not isolated because they involved
multiple departments.' While we recognize that you are developing and implementing corrective
actions to address this adverse trend of human performance events, the effectiveness of your
actions has yet to be demonstrated.
