Subject: Inspection No. 50-29/87-02

This transmits the January 27 - April 17, 1987 resident inspection findings by

Mr. H. Eichenholz of this office at the Yankee Nuclear Power Station, Rowe, Massa-

chusetts. Those findings were based on observation of activities, interviews, in-

dependent measurements, and document reviews, and have been discussed with

Mr. N. St. Laurent of your staff.

Based on the results of this inspection, it appears that one of your activities

was not conducted in full compliance with NRC requirements, as set forth in the

Notice of Violation, enclosed herewith as Appendix A. This violation has been

categorized by severity level in accordance with the " General Statement of Policy

and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement

Policy). You are required to respond to this letter and in preparing your response,

you should follow the instructions in Appendix A.

The violation (described in Appendix A) concerns the inadequate reviews, by both

the cognizant department supervisor and the shift supervisor, of a temporary change

request that permitted the incorrect removal of a main coolant pressure channel

from service, which resulted in the inadvertent actuations of a safety injection

and nonessential containment isclation systems.

Additionally, on March 18, 1987, Mr. Eichenholz provided Mr. B. Drawbridge of your

staff a detailed evaluation of licensee event report (LER) quality, Attachment I

to the enclosed inspection report, which was used as input to the Systematic As-

sessment of Licensee Performance of Yankee Nuclear Power Station for the period

from February 1, 1985 to October 6, 1986. The LER evaluation was made by the NRC

using the basic methodology presented in NUREG/CR-4178, March 1985.
