-stated that after the summer refueling outage, control rod number twenty four (24)
- was found to be disconnected from its drive shaft. The' reactor had been up to-Mode'2 while pre-start testing was being done. The reactor was put into Mode 5
=which did not constitute refueling operations (Mode 6).
In Mode 5, the reactor-head is in place and all studs are fully tensioned. The manipulator crane and
cthe universal handling tool;,are routinely used to latch onto control rods but-4 this requires the reactor to be in Mode 6 with the head removed. -With the-reactor head.in place however,- the proper location of the manipulator crane and the universal; handling tool was not feasibic.
In the November 3 1990-letter, Yankee outlined the approach for performing
- the' maintenance evolution to relatch the control rod. That maintenance effort-was'also discussed:in telephone conferences on. November 2, 1990 and November 3, 1990.- NRC evaluated the information provided in both the letter and telephone conference ils and concluded that your method for relatching the-rod was acceptable..
This was communicated on a phone call on November 3, 1990..
The maintenance operation was witnessed by Thomas Kosby NRC's Sr. Resident inspector'at YNPS;on November 4,1990 who reported that the maintenance operation was successful:and-the control rod was relatched.
