Subject: Inspection Report No. 50-29/90-16 .

This refers to the routine resident safety inspection conducted by Messrs. T. Koshy and

M. Markley of this office on August 21,1990 - October 1,1990, at the Yankee Nudear Power

Station, Rowe, Massachusetts. The inspection consisted of observation of activities, interviews

with personnel and document reviews. The results of the inspection are described in the NRC

Region I Inspection Report enclosed with this letter and were discussed with Mr. N. St. Laurent

at the conclusion of the inspection. l

Several observatioris during the ru;ent refueling outage are noteworthy. Your replacement of  !

the aging excore nuclear instrumentation system was considered a positive initiative. However,

.

!

problems noted during main coolant pump maintenance and the resultant occupational radiation

exposure warrant further management attention. In addition, the presence of a temporary

scaffold in the pressurizer cubicle and other transient materials in the vapor container during ,

. plant operation is of concern. Also, our inspectors verified t%t your staff completed an

observation of fuses installed in the containment; however, no assessment was made as to the I

adequacy of the installed fuses.

Inadequate corrective actions to preclude repetitive emergency lighting failures resulted in a

violation of 10 CFR 50, Appendix B, quality assurance requirements. You are required to  !'

respond to this letter and, in so doing, should follow the instructions in Appendix A. In addition

to your response to the Notice of Violation, please describe your actions to address the additional

concerns noted above.
