/ 1.0 PURPOSE -

'd This instruction provides direction and accept / reject criteria 1.1 to be applied when performing documentation review of Field Fabricated Tanks.

2.0 APPLICABILITY 2.1 This procedure applies to all Field Fabricated Tanks or portion of a tank contained in the Field Fabricated Tank Population for Unit 1, Common and Unit 2 which were not accessible for reinspection. The population is described in the Population Description for Field Fabricated Tanks.

3.0 REFERENCES

3.2 CPP-009, " Performance of Reinspections and Documentation Reviews."

4.0 GENERAL 4.1 Responsibilities QA/QC Inspector performs a documentation review in accordance with established project procedures and instructions.

Reference 3.2 addresses the method used to perform reinspection and to document and process the results.

4.2 Policy Activities performed under this procedure shall conforn to the policies contained in the latest Comanche Peak Response Team l

Program Plan and Issue Specific Action Plan VII.c.

5.0 PROCEDURE NOTE: The number before each Attribute corresponds to the Checklist Attribute number.

A. Tank Diameter (For Flat Bottom Tanks)

Verify that the difference between the maximum and minimum diameter readings is less than or equal to 1%

of the average diameter. All of the diameter measurements are to be found on CBI FORM DRP-3, Tank Diameter Checks. (See Attachment 6.2)

B. Shell Diameter (For Horizontal Tanks)

Verify that the difference between the maximum and minimum diameters from each section location does not exceed 11 inches. Measurements are to be found on CBI form DR1 Shell Diameter Checks. ( See Attachment 6.3)

C. Vessel Head Diameter (For Horizontal Tanks)

Verify that the difference between the maximum and minimum vessel head diameter readings does not exceed 11 inches. The Vessel Head diameter readings are recorded on CBI FORM DR2, Vessel Head Dimensional s Checks (See Attachment 6.4)

D. Temporary Foundation Level Verify that the 10 temporary foundation levelness readings are recorded and that the difference between any six (6) sequential readings is less than or equal to 1/8 inch, and that the difference between the maximum and minimum readings is less than or equals to 1/4 inch. Temporary foundation levelness readings are recorded on CBI FORM DRP-1, Level Check of Temporary Foundation (See Attachment 6.5)

E. Tank Bottom Crown Verify that the tank bottom crown reading is recorded and that the difference between crown elevation and foundation elevations is as shown on the CBI General Plan drawing. The tolerance for this difference in Elevation is i 1 inch. The Crown Reading can be found on CBI FORM DRP-2, Sand Grade Checks (See Attachment 6.6) For Foundation elevations use CBI FORM DRP-1.

A. Horizontal Tanks Verify that the face of the recording chart is signed by the CBI Welding and Quality Assurance Supervisor and that the hydrostatic test pressure (minimum 24PSIG) was held for a minimum of 10 minutes.

B. Flat Bottom Tanks.

Verify that the CBI Welding and Quality Assurance Supervisor signed the hydrotest data sheet.

3. Record Drawing NOTE: The Record Drawings are found in the CBI Contract Records File, classification no. 8.5 (See attachment 6.7 Record Drawing, and 6.8 Record Drawing Table for example)

A. Joint Identification s Verify that for each weld joint shown on the Record Drawing a unique identification number is given.

5 B. Welders Identification Verify that for each weld joint shown on the Record Drawing a welders identification is given.

l C. Welding Procedure Specification t

Verify that the welding procedure specification (WPS) used is identified on the Record Drawing Table for each weld joint shown on the Record Drawing.

D. Record Drawing Table Signoffs Verify that for each weld joint shown on the Record Drawing the " Fit Up Checked", " Welding Checked", and the " Finished Joint Checked" spaces on the Record Drawing Table have been signed and dated.

E. NDE Procedures Identified Verify that on the Record Drawing Table the NDE Procedure used is identified, and the evaluated accepted results are dated and initialed.

4. Welding Materials Verify that the welding materials used for each veld joint were correct. The Welding and Quality Assurance Supervisor's initials in the " Welding Checked" space on the Record Drawing Table is the official documentation that the proper welding material was used.
5. Procedure Approval NOTE: This information may be obtained from the Contracts Administration's Contract Files Located in Warehouse A.

Verify that each Welding Procedure Specification, Repair Procedure Specification, Repair Procedure and Nondestructive Examination Procedure used has been approved for use at the CPSES by Texas Utilities Services Inc. or Gibbs and Hill.

6. Welder and Welding Operator Qualifications Verify that each welder or welding operator was qualified to f'~s perform the welding operation. Use the Record Drawings and

() the Record Drawing Tables to obtain the welders symbol, welding procedure specifications, and applicable dates when the welding was performed. Use the Welder and Welding Operator Performance Qualification Tests (See Attachments 6.10 and 6.11) to verify that the Welder or Welding Operator was certified to perform the specified welding operation.

NOTE: For proof of qualification one of the following is

, required:

A. A copy of the welder's/ welding operator's qualification test records, examples of which are Attachments 6.10 and 6.11 with the following statement, "This welder / welder operator has maintained his qualification in the listed process (last welded on date) signed and dated by the Construction Welding and QA Manager within the previous 90 days of the welding operation.

B. A copy of the welder's/ welding operator's certified qualification test dated within the previous 90 days of the welding operation.

C. A copy of the welder's/ welding operator's certified qualification which has been kept current.

7. Nondestructive Examinations (NDE)

NOTE: a. NDE Reports may be found in the CBI Contract File, Records classification nos. 7.3 thru 7.6.3.

b. NDE Personnel Certifications may be found in the CBI Contract Records File, classification no. 8.12.

A. Weld Joint Identification Verify that the NDE Report (see Attachment 6.12) has the weld joint identified and is traceable to the joint identification shown on the Record Drawing.

B. NDE Procedure / Report Identification Verify that on each NDE Report the NDE Procedure and Revision used is identified.

C. Reviewer Certification Verify that a certified NDE evaluator / interpreter signed i.e., Applicable Certification Level on the NDE s Report.

8. Nondestructive Examination (NDE) Personnel Certifications NOTE: NDE Personnel Certification may be found in the CBI Contract Records File classification, no. 8.12.

A. NDE Personnel Certification Verify that each NDE person that performed NDE as identified on the NDE Reports, was certified to perform the NDE Procedures used prior to the date of the NDE.

B. Vision Test Record i

Verify that each NDE person had an annual NDE Certification vision test recorded. This can be verified by reviewing the CBI NDE Certification Annual Vision Test Record. ( See Attachment 6.13)

C. NDE Certification Verify that the CBI NDE Performance Qualification and Certification form (Attachment 6.14) is signed and dated in the " Administered By" or " Verified By" space, by a certified individual and that the certification statement signature blanks are signed.

9. Seismic Restraint As-Built Dimensions Note: The As-Built Drawings may be found within the CBI Contract Records File classification no. 4.1.5.

Verify that the As-Built Drawing shows the elevation of the Seismic Brackets within il inch of the measurement of the Seismic Brackets recorded on QI-041, Attachment 6.7.

10. Material Verification Verify that the Metal Material Verification Summary Sheet, (See Attachment 6.15), identifies the correct material (See material requirements on Certified Drawings) and that the "CTR Checked" space is signed.
