l 1.0 PURPOSE This procedure defines the requirements for procedures to be used to implement the DAP Guolity Assurance Program and to execute the Design Adequacy Program. It also defines the requirements for the development, review, opproval, implementation, and control of those procedures.

2.0 SCOPE This procedure oddresses procedures applicable to the entire DAP scope and detailed instructions for specific disciplines required for the performance of the DAP work scope. Procedures opplicable to overall CPRT octivities are included in the CPRT Procedures Manual.

3.0 DEFINITIONS AND RESPONSIBILITIES 3.1 Definitions 3.1.1 DAP Procedures DAP Procedures provide the basic instructions for performance of DAP work.

These procedures implement the DAP and those parts of the CPRT procedures that apply to the DAP.

3.1.2 DAP Discipline Instructions DAP Discipline Instructions (Dis) supplement the DAP Procedures by providing detailed instructions required for performance of work by specific DAP Discip-lines or by specifying allowable alternatives to the procedural requirements in the DAP.

Together, the DAP disciplines comprise all work to be performed by the DAP. The following is a list of the disciplines and their abbreviations, o Piping / Supports (P) o Civil / Structural (C/S) o Programmatic / Generic Implications (PGI) o Mechanical Systems (M) o Electrical and Instrumentation & Control (EIC) 3.1.5 Procedures Manuals Procedures Manuals are complete sets of current DAP Procedures and complete sets of dis developed for individual disciplines. Eoch manual includes a table of contents that lists each procedure number, title, and revision status.

4.0 INSTRUCTION 4.1 Genero!

4.1.1 Table of Contents A table of contents shall be used for each DAP Procedure and DI. It shall be inserted immediately following the document cover sheet. it shall identify page numbers for the following:

o Document cover sheet (Poge I oil) o Table of Contents (Page 2 of,X_)

o Standardized Sections 1.0 through 3.0 (See Appendices A and B) o All appendices (pages numbered A-1, B-1, etc.).

4.1.2.2 Standard Page Format The body of each procedure shall be prepared using the standard form included as Attachment E to this procedure. Discipline Instructions are prepared on a Q

U similar form (Attachment F). g 4.1.3 Revisions Procedure revisions shall generally be mode by revising the entire procedure with vertical lines at the right hand margin of each page to indicate changes. For very simple or limited change, addendo may be used in lieu of complete revision.

Such cases would typically be limited to about two lines on each of two pages. A maximum of two oddendo are permitted between complete revisions. Eoch offected page shall be identified as to the controlling oddendum. Revisions and oddendo shall be developed and controlled in occordance with the procedure specified for the offected procedure per Sections 4.2 or 4.3, as applicable. The requirements for review and opproval of revisions and addendo are identical to those for the original issue.

DEVELOPMENT Ato USE OF DAP Revision: 3 PROCEDURES AND DISCPLIE INSTRUCTIONS When the need for o procedure or revision is identified, the DAP Monoger either prepares the procedure or revision, or assigns its development to someone else.

For original procedures, he assigns a procedure number by entering a new number and title in the Procedures Index. He subsequently monitors development of the procedure through its approval.

4.2.2 Procedure Preparation The person assigned prepares the procedure using the format of Attachment A as guidance. In general, the format of Attachment A shoulc' be followed, but the DAP Monoger may allow variations to allow for specific needs. Interfaces and required interfoce actions are explicitly identified in the procedure. The procedure should also include acceptance criteria when oppropriate.

O 4.2.3 Procedure Review As the procedure or procedure revision nears completion, a reviewer is assigned and a draft copy is distributed. Normally, the DAP Monoger provides this review, but he may delegate this responsibility. The reviewer ensures that the

! procedure is complete, meets program requirements, and is consistent with program logic. After agreement between the preparer and reviewer, o final l revision is prepared. The preparer and reviewer sign the appropriate portions of i

the procedure cover sheet hee Attachment D), which is forwarded with the procedure to the DAP Monoger for review.

l 4.2.4 DAP Manager Review The DAP Monoger review includes review of all interface requirements. When satisfied that the procedure meets CPRT and DAP requirements in a manner consistent with program logic, he recommends opproval to the' Review Team Leader. 4.2.5 Review Team Leoder Approvo!

The Review Team Leader reviews and indicates opproval of the procedure by signing and dating the cover sheet. The opproved procedure is returned to the DAP Monoger.

4.2.6 Procedure issuance The DAP Monoger files a copy of the procedure in the Master Procedures Manual, updates the table of contents, and files the Distribution Sheet (Attach-ment C) in the DAP Procedure Distribution Log. He distributes the procedure by memorandum to the individuals listed below. The receptent returns the receipt ocknowledgement to the DAP Manager. The receipt ocknowledgements are filed in the DAP Procedure Distribution Log with the Distribution Sheet for the specific procedure:

PROCEDURES AND DISCIPLIFE INSTRUCTIONS l 4.3 Discipline instructions 4.3.1 Instruction Identificotton As the need for o discipline instruction or revision is identified, the Discipline Coordinator for the offected DAP discipline assigns responsibility for its development to on Individual. For original procedures, he assigns number and title in the Instruction Index. He subsequently monitors development of the instruction through its opproval.

4.3.2 Instruction Preporation The assigned person prepares the instruction using the format of Attochment B os guidance. In general, the format of Attachment B should be followed, but the DAP Monoger may allow vorlations to allow for specific needs. Interfoces and required interfoce octions are explicitly identified in the instruction. The instructions should also include acceptance criteria when oppropriate.

4.3.3 Instruction Review l

l As the document revision nears completion, o reviewer is ass *gned and a draft copy is distributed. The reviewer ensures that the instruction is complete, meets requirements of the DAP Procedures, and is consistent with program logic.

Normally, the Discipline Coordinator provides this review, but he may delegate this responsibility. After ogreement between the preparer and the reviewer, a final revision is prepared. The preparer and reviewer sign the oppropriate l

portions of the instruction cover sheet (see Attachment D) which is forwarded l with the instruction to the Discipline Coordinator for review.

When satisfied that the instruction meets DAP requirements in a manner consistent with program logic, the Discipline Coordinator recommends opproval to the DAP Monoger. The Discipline Coordinator indicates this recommendation by signing and dating the cover sheet. if the Discipline Coordinator performed both of the reviews required by 4.3.3 and 4.3.4, only one signature is required.

d 4.3.5 DAP Monoger Approval The DAP Monoger reviews and indicates opproval of the instruction by signing and dating the cover sheet.

4.3.6 Instruction issuance The DAP Monoger files a copy of the instruction in the Discipline Instruction Manual, updates the table of contents, and files the Distribution Sheet (Attach-ment C) in the Discipline Instruction Distribution Log. DEVELOPMENT APO USE OF DAP Revision: 3 PROGDURES AND DISCPLIE INSTRUCTIONS o Other identified interface contact personnel o Others os determined by the DAP Monoger and Discipline Coordinator o Appropriate files 4.4 Interim Use Procedures The Design Adequocy Monoger may, when he deems it oppropriate, direct that a procedure or procedure revision be implemented on on interim use basis.

Situations where interim use procedures are appropriate include, but are not limited to, those circumstances where o period of " trial use" is necessary to test the odequocy of a procedure or where changed conditions require the immediate issuance of a procedure prior to completion of the approval process required by O this procedure. A procedure that is issued for interim use shall be clearly marked as such and the transmittal memorandum shall otso clearly state that the procedure is for interim use only. An interim use procedure shall be effective for a period not to exceed 30 days from the date of the transmittal memo. All I work performed under on interim use procedure shall be brought into conform-once with the permanent replocement procedure within 30 days cf its approval.

Becosse work dme under on interim use procedure is brought into conformance with the permanent procedure, it is not necessary that a copy of the interim use i procedure be retained in the DAP files. A controlled Distribution Log shall be l maintained for each procedure issued for interim use and each person receiving on interim procedure shall receive a copy of the permanent procedure.

4.5 EFFECTIVE DATES Original issuances of DAP procedures and Discipline Instructions become effec-tive on opproval unless otherwise stated in the procedure or instruction. The DAP Monoger shall provide direction regarding the extent to which newly issued O

Unless such direction is provided, procedures shall not be required to be applied retroactively. Discipline Coordi-notors shall provide direction regarding retroactive opplication of Discipline Instructions. The foregoing shall not prevent the retroactive opplication of a procedure (that is not required to be opplied retrooctively) should the user of the procedure deem it oppropriate to do so.

Revised procedures (including oddendo to procedores) become effective five (5) working days offer opprovat unless otherwise stated in the procedure. Unless the procedures specifically requires retrooctive opplication of the revision, the following shall opply:

o For work completed (approved) prior to the opproval date of the O' revised procedure, no revision to the work shall be necessary unless otherwise directed by the DAP Monoger.

o For work comptefed (approved) within 30 calendor days of the revised procedure opproval date, the offected work may be completed using either the superceded procedure or the revised procedure. <

o For werk that was previously completed under o superceded revision of a procedure and that is being revised (when a revised procedure is in effect), the revision of the work shall include revisions necessary to bring the work into conformance with the revised procedure. The DAP Monoger may grant exemptions to this requirement.

o For work completed more than 30 calendor days offer the opproval a date of a revised procedure, the work shall be brought into confor-mance with the revised procedure prior to its approval unless on exemption is granted by the DAP Monoger.

5.0 DOCUMENTATION l 5.1 Distribution Log Attachment C is a sompte page of the Procedures / Instruction Distribution Log.

One such sheet shall be filed in the log for each DAP Procedure and Discipline Instruction defined. Implementation is in occordance with Section 4.2.6 for DAP Procedures and Section 4.3.6 for Discipline Instructions. Separate logs for procedures and instructions are used.

5.1.1 DAP Procedure Numbers DAP Procedure numbers are composed of the prefix "DAP" followed by a O sequential number.

5.l.2 Discipline Instruction Numbers Discipline Instruction numbers are composed of the prefix "Dl" followed by on abbreviation representative of the originating discipline (see Section 3.1.4) i followed by a sequential number.

5.2 Procedure Cover Sheets Attachment D is a cover sheet form which is used for each DAP Procedure or Discipline Instruction to be developed. Use of the form is in occordance with Sections 4.2.3 and 4.2.5 for DAP Procedures and Sections 4.3.3 and 4.3.5 for Discipline instructions.
