'Changs Notica 001 CPP-003 Revision: 3 1.

Revise Section 5.4.1, in part, to read "...for...." in lieu of

"... prior to...."

2.

Revise the footnote (indicated by an asterisk) on page seven to read: "*The Braidwood Construction Assessment Program."

3.

Revise the first paragraph (after the note) on page 9 to. read:

"The certification may be conditional and/or restricted. That is, certification may be conditional upon the completion of the verification of the education and experience.

A NOTE: Should it be subsequently determined that an individual is unqualified, all of his/her work shall be reinspected and reviewed.

The certifying individual may also qualify the'ertification in the C

" LIMITATIONS OF CERTIFICATION" portion of the Personnel Certification form."

4.

Add Section 5.4.7 after the second paragraph of Section 5.4.6 as follows:

"5.4.7 Recertification 0

The CA ensures that inspection, examination, and test personnel are recertified at least annually.

Recertification is contingent upon the recommendation of the appropriate Level III inspector and compliance with any established requirements."
