TRAINING AND QUALIFICATIONS PURPOSE This addendum allows the use of forms other than those specifically included in the procedure provided that the alternative form contains equivalent information .

SCOPE To Sections 4.2 and 5.0 the following is added:

l Alternative forms may be used in lieu of those ottoched to this procedure, d provided that the alternative form contains equivalent information to the standard form.

INSTRUCTION i

This addendum is retroactive to April 1,1986, and shall remain in effect until opproval of the next revision of DAP 15. This addendum shall be securely attached to each controlled copy of DAP 15.
